目的:分析研究影响青年高血压脑出血患者手术预后的危险因素。方法回顾37例行开颅手术的青年高血压脑出血患者临床资料,根据术后1个月时的格拉斯哥昏迷评分( GCS)及死亡情况,将GCS评分小于等于术前及死亡的病例作为未好转组,GCS评分大于术前者作为好转组,统计性别、年龄、术前GCS评分、体重、出血位置、是否破入脑室、是否合并脑疝、出血量、最高收缩压、血肿清除量等方面与手术预后的关系。结果术前GCS评分、出血量、破入脑室、合并脑疝、术前最高收缩压等因素为影响手术预后的独立危险因素。结论出血前积极控制血压,出血后对符合手术指征者争取早期手术治疗可能会有较好的预后。

목적:분석연구영향청년고혈압뇌출혈환자수술예후적위험인소。방법회고37례행개로수술적청년고혈압뇌출혈환자림상자료,근거술후1개월시적격랍사가혼미평분( GCS)급사망정황,장GCS평분소우등우술전급사망적병례작위미호전조,GCS평분대우술전자작위호전조,통계성별、년령、술전GCS평분、체중、출혈위치、시부파입뇌실、시부합병뇌산、출혈량、최고수축압、혈종청제량등방면여수술예후적관계。결과술전GCS평분、출혈량、파입뇌실、합병뇌산、술전최고수축압등인소위영향수술예후적독립위험인소。결론출혈전적겁공제혈압,출혈후대부합수술지정자쟁취조기수술치료가능회유교호적예후。
Objective To study different factors impacting on the outcome of surgery in hypertensive intracerebral hemorrhage in young adults. Method Between August 2011 and August 2014, clinical data of 37 hypertensive intracerebral hemorrhage in young adults were collected in neurosurgical department of Beijing Jingmei Group Gener-al Hospital. Surgical outcome was evaluated with one-month GCS score and survival status. The relationship among age, sex, preoperative GCS score, weight, location of hemorrhage, hemorrhage in ventricle, brain hernia, hemor-rhage volume, Maximum systolic pressure, hemorrhage removal ratio were assessed. Result That preoperative GCS score, hemorrhage in ventricle, brain hernia, hemorrhage volume and maximum systolic pressure are factors to affect the outcome of operation. Conclusion controlling blood pressure before hemorrhage and performing operation in an early stage according to the surgical indications when hemorrhage occurred predicts a better outcome.
