目的：探讨负压封闭引流（VSD）技术在下肢慢性难愈性皮肤溃疡修复中的应用价值。方法：对45例下肢皮肤慢性溃疡患者采用一期手术清创、负压封闭引流覆盖创面（必要时二次或多次手术），二期手术通过中厚皮片或全厚皮片移植修复创面。结果：45例皮肤慢性溃疡患者经过一期手术清创负压封闭引流治疗后，创面坏死组织全部清除，鲜嫩肉芽覆盖外露的肌腱、骨及软骨组织。通过二期手术在鲜嫩肉芽上植皮，皮片全部存活，慢性溃疡创面得以成功修复。结论：负压封闭引流用于治疗下肢慢性难愈性皮肤溃疡，手术操作简便，疗效显著，值得推广。
목적：탐토부압봉폐인류（VSD）기술재하지만성난유성피부궤양수복중적응용개치。방법：대45례하지피부만성궤양환자채용일기수술청창、부압봉폐인류복개창면（필요시이차혹다차수술），이기수술통과중후피편혹전후피편이식수복창면。결과：45례피부만성궤양환자경과일기수술청창부압봉폐인류치료후，창면배사조직전부청제，선눈육아복개외로적기건、골급연골조직。통과이기수술재선눈육아상식피，피편전부존활，만성궤양창면득이성공수복。결론：부압봉폐인류용우치료하지만성난유성피부궤양，수술조작간편，료효현저，치득추엄。
Objective To explore the application value of the vacuum sealing drainage (VSD) in reconstruction of chronic and refractory skin ulcer of lower limb. Methods 45 patients of the chronic skin ulcers of lower limb were treated by covering vacuum sealing drainage after one- stage debridement operation(When necessary, two or more operations), and repaired by the Split thickness skin graft or Full thickness skin graft in the two- stage operation. Results All of necrotic tissue of 45 patients were debrided, and Tendons, bones and cartilages were covered by fresh granulation , after one- stage treatment of vacuum sealing drainage. The skin graft survived and the chronic ulcer wounds were successfully repaired after the fresh granulation was translanted into it in the two- stage operation. Conclusion The vacuum sealing drainage used for chronic and refractory skin ulcer of lower limb is simple and effective in treatment, which is very worthy of spreading.
