目的：探讨新疆维吾尔族、汉族鼻咽癌患者外周血潜伏膜蛋白2 A（LMP2 A）特异性T细胞免疫反应的差异性。方法选择114例鼻咽癌初治患者（维吾尔族48例，汉族66例）及120例健康志愿者（维吾尔族50例，汉族70例），治疗前抽取肘静脉血，分离外周血单个核细胞，采用酶联免疫斑点法（ELISPOT）检测外周血中LMP2 A特异性T细胞（CTL）水平。比较鼻咽癌患者与健康志愿者、维吾尔族患者与维吾尔族健康志愿者、汉族患者与汉族健康志愿者、维吾尔族患者与汉族患者外周血 LMP2 A特异性 CTL频数的差异。结果鼻咽癌患者的外周血 LMP2A特异性CTL水平明显低于健康志愿者（P ＝0．0316）；维吾尔族鼻咽癌患者和汉族鼻咽癌患者分别与同民族健康志愿者比较，外周血 LMP2 A特异性 CTL频数均低于同民族健康志愿者（P ＝0．0329，P ＝0．0479），差异有统计学意义；汉族鼻咽癌的外周血中 LMP2A特异性 CTL水平亦高于维吾尔族鼻咽癌患者，差异有统计学意义（P ＝0．0227）。结论鼻咽癌患者对于 LMP2A特异性的T细胞免疫反应明显低于健康人群，这可能是 EBV潜伏感染，最终导致鼻咽癌发生的重要原因之一。
목적：탐토신강유오이족、한족비인암환자외주혈잠복막단백2 A（LMP2 A）특이성T세포면역반응적차이성。방법선택114례비인암초치환자（유오이족48례，한족66례）급120례건강지원자（유오이족50례，한족70례），치료전추취주정맥혈，분리외주혈단개핵세포，채용매련면역반점법（ELISPOT）검측외주혈중LMP2 A특이성T세포（CTL）수평。비교비인암환자여건강지원자、유오이족환자여유오이족건강지원자、한족환자여한족건강지원자、유오이족환자여한족환자외주혈 LMP2 A특이성 CTL빈수적차이。결과비인암환자적외주혈 LMP2A특이성CTL수평명현저우건강지원자（P ＝0．0316）；유오이족비인암환자화한족비인암환자분별여동민족건강지원자비교，외주혈 LMP2 A특이성 CTL빈수균저우동민족건강지원자（P ＝0．0329，P ＝0．0479），차이유통계학의의；한족비인암적외주혈중 LMP2A특이성 CTL수평역고우유오이족비인암환자，차이유통계학의의（P ＝0．0227）。결론비인암환자대우 LMP2A특이성적T세포면역반응명현저우건강인군，저가능시 EBV잠복감염，최종도치비인암발생적중요원인지일。
Objective To study the differential immune reaction of the LMP2A spicific T cell in Han and Uyghur nasopharyngeal carcinoma patients in Xinjiang.Methods 114 Nasopharyngeal Carcinoma patients (Uyghur 48,Han 66)and 120 healthy donors (Uyghur 50,Han 70)were included in the study.10 ml blood were taken before any treatment,PBMCs were separated and LMP2A specific T cell responses were measured by using ELISPOT assay.Compare the differences between nasopharyngeal carcinoma patients and healthy donors,Uyghur nasopharyngeal carcinoma patients and Uyghur healthy donors,Han naso-pharyngeal carcinoma patients and Han healthy donors,Uyghur nasopharyngeal carcinoma patients and Han nasopharyngeal carcinoma patients.Results We found significant difference of LMP2A specific T cell responses between NPC patients and healthy donors (P=0.031 6).We also found the significant difference of LMP2A specific T cell responses in another 3 groups:Uyghur NPC patients and Uyghur healthy donors (P =0.032 9),Han NPC patients and Han healthy donors (P =0.047 9),Uyghur and Han NPC patients (P =0.022 7).Conclusion The lower immune reaction of the LMP2A spicific T cell in nasopharyngeal carcinoma patients may be one of the most important reasons about latent EBV infection,and eventually lead to the occurrence of nasopharyngeal carcinoma.
