目的:探讨能谱CT成像(GSI)技术对于甲状腺乳头状癌(PTC)的诊断价值。方法收集经手术病理证实的32例(共45个结节)甲状腺乳头状癌患者,行甲状腺能谱CT单能模式平扫及增强扫描,测量结节的碘浓度,计算能谱曲线斜率,并与病理结果对照。结果平扫期、动脉期、静脉期甲状腺乳头状癌的碘浓度(100μg/ml)分别为5.77±4.23、10.42±5.78、18.27±5.09;能谱衰减曲线斜率分别为-0.53±0.26、-0.84±0.52、-1.24±0.30。各期乳头状癌组的碘浓度及曲线斜率绝对值均小于良性结节组和正常甲状腺组(P均<0.05)。32例中有23例出现淋巴结转移,其中18例能谱曲线与原发灶完全一致。结论能谱CT对甲状腺乳头状癌的准确诊断具有重要价值。

목적:탐토능보CT성상(GSI)기술대우갑상선유두상암(PTC)적진단개치。방법수집경수술병리증실적32례(공45개결절)갑상선유두상암환자,행갑상선능보CT단능모식평소급증강소묘,측량결절적전농도,계산능보곡선사솔,병여병리결과대조。결과평소기、동맥기、정맥기갑상선유두상암적전농도(100μg/ml)분별위5.77±4.23、10.42±5.78、18.27±5.09;능보쇠감곡선사솔분별위-0.53±0.26、-0.84±0.52、-1.24±0.30。각기유두상암조적전농도급곡선사솔절대치균소우량성결절조화정상갑상선조(P균<0.05)。32례중유23례출현림파결전이,기중18례능보곡선여원발조완전일치。결론능보CT대갑상선유두상암적준학진단구유중요개치。
Objective To explore the value of Gemstone spectral imaging(GSI)in diagnosis of papillary thyroid carcinoma. Methods 32 patients with 45 pathologically confirmed papillary thyroid carcinoma nodules underwent dynamic contrast-enhanced thyroid GSI.The iodine concentrations and slopes of the spectral curves of the lesions were calculated and correlated with pathology. Results The tumors had significantly(P<0.05)lower iodine concentrations and absolute values of the slopes of spectral curves in the unenhanced(5.77±4.23μg/ml,-0.53±0.26),arterial(10.42±5.78μg/ml,-0.84±0.52),and venous(18.27±5.09μg/ml,-1 . 2 4 ± 0 . 3 0) phases than that of benign nodules and normal tissues.Of 32 patients,23 had lymph node metastasis with spectrum curves identical to the primary tumors in 18.Conclusion GSI is valuable in accurate diagnosis of papillary thyroid carcinoma.
