目的：探讨高强度聚焦超声（ HIFU）治疗良性前列腺增生症（ BPH）的临床意义及疗效观察。方法：对303例不能耐受手术又不愿接受膀胱造瘘术的BPH患者行HIFU治疗。结果：303例BPH 患者中，治愈207例，明显缓解53例，轻度改善34例，无效9例，总有效率97．03％。治疗后IPSS、Qmax、Volume、PVRV明显优于治疗前，各项差异均有统计学意义，治疗中所有患者均出现血尿，耻骨联合处隐痛72例，均未影响治疗，2周内症状消失，急性附睾炎7例，给予抗感染处理后治愈。结论：HIFU对治疗不能耐受手术的老年BPH患者是一种无创、无痛、安全有效的治疗方法，近期疗效显著，远期效果有待进一步观察。
목적：탐토고강도취초초성（ HIFU）치료량성전렬선증생증（ BPH）적림상의의급료효관찰。방법：대303례불능내수수술우불원접수방광조루술적BPH환자행HIFU치료。결과：303례BPH 환자중，치유207례，명현완해53례，경도개선34례，무효9례，총유효솔97．03％。치료후IPSS、Qmax、Volume、PVRV명현우우치료전，각항차이균유통계학의의，치료중소유환자균출현혈뇨，치골연합처은통72례，균미영향치료，2주내증상소실，급성부고염7례，급여항감염처리후치유。결론：HIFU대치료불능내수수술적노년BPH환자시일충무창、무통、안전유효적치료방법，근기료효현저，원기효과유대진일보관찰。
Objective Clinical significance and therapeutic efficacy of high intensity focused ultrasound in the treatment of benign prostatic hyperplasia .Methods In 303 cases of intolerance and unwilling to accept opera-tion bladder fistulization in HIFU patients treated with BPH .Results In 303 BPH patients , 207 cases were cured, 34 cases markedly relieved in 53 cases, slightly improved, 9 cases ineffective, the total efficiency of 97.03%.IPSS, Qmax, Volume, PVRV significantly effect after than before treatment , the difference was statisti-cally significant, in the treatment of all patients were hematuria , pubic symphysis pain in 72 cases, were not affect-ed by treatment, symptoms disappeared within 2 weeks, 7 patients with epididymitis, give the anti infection treat-ment cured after .Conclusion HIFU for surgical treatment can not be tolerated in the elderly patients with BPH is a non-invasive, painless, safe and effective method of treatment , a significant short-term effect, the long-term effect needs further observation .
