目的:探讨吸入伊洛前列素和一氧化氮治疗新生儿肺动脉高压的临床疗效及护理对策。方法选取肺动脉高压新生儿50例,随机分为研究组和对照组,各25例。两组患儿均给予基础治疗和一氧化氮吸入治疗,研究组患儿加用伊洛前列素雾化吸入。比较两组患儿的气道压力、血气指标和血流动力学指标。结果治疗后,两组患儿平均气道压力均显著降低,研究组低于对照组;氧合指数和血氧饱和度均显著提高,研究组显著高于对照组;肺动脉收缩压均显著降低,研究组显著低于对照组。以上比较,差异均有统计学意义( P<0.05)。两组患儿治疗前后血压比较,差异无统计学意义( P>0.05)。结论新生儿肺动脉高压患儿吸入伊洛前列素和一氧化氮可有效降低气道压力和肺动脉收缩压、提高氧合水平,合理的护理方法能进一步提高疗效。

Objective To study the clinical efficacy and nursing strategy of the iloprost and NO inhalation for treating neonatal pul-monary hypertension. Methods A total of 50 neonates with pulmonary hypertension were selected and randomly divided into the study group and the control group, 25 cases in each group. The two groups were given the therapy of the basic treatment and nitric oxide ( NO ) inhalation, while the study group was added with the iloprost nebulization inhalation. The airway pressure, blood gas indices and hemodynamic indices were compared between the two groups. Results The mean airway pressure after treatment in the two groups was significantly decreased;the oxygenation index and blood oxygen saturation were significantly increased, and the data in the study group were significantly higher than those in the control group;the pulmonary artery systolic pressure after treatment in the two groups was significantly reduced, but the data in the study group was significantly lower than that in the control group. The above differences be-tween the two groups had statistical significance ( P<0. 05 ) . The blood pressure before and after treatment had no statistical difference between the two groups ( P>0. 05 ) . Conclusion The iloprost and NO inhalation can effectively reduce the airway pressure and pul-monary arterial systolic pressure, improve the oxygenation level. The rational nursing method can further increase the efficacy.
