目的:探讨先天性心脏病合并感染性心内膜炎的诊断与治疗方法。方法选取2009年1月至2012年6月入院确诊治疗的先天性心脏病合并感染性心内膜炎的患者86例,回顾性分析其临床资料,对86例患者行超声心动图检查并且对其进行血及赘生物培养、药物敏感性试验,观察病原体种类、心动图表现等。结果86例患者共行97例血液培养,致病菌种类最多的为草绿色链球菌,其次为口腔链球菌、金黄色葡萄球菌等。治疗后,86例患者显效24例(27.91%),好转48例(55.81%),无效14例(16.28%),总有效率为83.72%,无死亡病例。所有患者随访6个月后,均恢复正常工作学习,心功能恢复到Ⅰ~Ⅱ级。患者的生活质量与治疗前相比均有很大提高。结论先天性心脏病合并感染性心内膜炎的患者通过超声诊断及血液培养可以诊断;在治疗中先采取抗生素治疗,效果不良的情况下应及时采取相应的手术治疗。

목적:탐토선천성심장병합병감염성심내막염적진단여치료방법。방법선취2009년1월지2012년6월입원학진치료적선천성심장병합병감염성심내막염적환자86례,회고성분석기림상자료,대86례환자행초성심동도검사병차대기진행혈급췌생물배양、약물민감성시험,관찰병원체충류、심동도표현등。결과86례환자공행97례혈액배양,치병균충류최다적위초록색련구균,기차위구강련구균、금황색포도구균등。치료후,86례환자현효24례(27.91%),호전48례(55.81%),무효14례(16.28%),총유효솔위83.72%,무사망병례。소유환자수방6개월후,균회복정상공작학습,심공능회복도Ⅰ~Ⅱ급。환자적생활질량여치료전상비균유흔대제고。결론선천성심장병합병감염성심내막염적환자통과초성진단급혈액배양가이진단;재치료중선채취항생소치료,효과불량적정황하응급시채취상응적수술치료。
Objective To study the diagnosis and treatment methods of congenital heart disease ( CHD ) complicating infective endocarditis. Methods 86 cases of CHD complicating infective endocarditis in our hospital from January 2009 to June 2012 were selected and per-formed the retrospective analysis on the clinical data. The 86 cases were given the echocardiographic examination, blood and vegetations cultures and susceptibility testing. The types of pathogens and echocardiography performance were observed. Results The 86 cases were performed 97 case-times of blood culture, the most types of pathogens were Streptococcus viridans, followed by oral streptococci, Staphylo-coccus aureus, and so on. Among the 86 cases after treatment, 24 cases ( 27. 91% ) were significantly effective, 48 cases ( 55. 81% ) were improved, 14 cases ( 16. 28% ) were ineffective, the total effective rate was 83. 72%, without death case. All cases recovered to normal work and study after 6-month follow-up, and he heart function recovered to the grade Ⅰ-Ⅱ. In addition, the patient ' s quality of life was greatly improved compared with before treatment. Conclusion CHD complicating infective endocarditis can be diagnosed by ul-trasound and blood culture;its treatment firstly adopts the antibiotic therapy, under the condition of poor effect, the corresponding opera-tive symptomatic treatment should be taken in time.
