[目的]探讨缝合器在复杂主动脉病变病人行经皮主动脉腔内修复术应用的护理。[方法]入选2011年9月—2013年3月在我科住院的复杂主动脉病变病人共13例,对14处穿刺部位进行预先埋置缝合技术,按照标准方法行造影及覆膜支架植入,对术后所有病人止血成功率、出血血肿、假性动脉瘤、感染率、住院时间、下肢深静脉血栓形成、下肢制动时间进行统计。[结果]止血成功率为100%；术后并发血肿1例,经处理后血肿消退,无假性动脉瘤发生,无术后感染。住院时间9.4 d±3.3 d,无下肢深静脉血栓形成,下肢制动时间明显缩短至4 h~6 h,病人卧床引起腰背部等不适感明显减少。[结论]在实施复杂主动脉病变的腔内修复术中,如无其他禁忌证,使用 Perclose Proglide 缝合器预先缝合穿刺口技术是安全有效的,配以专业的护理可缩短病人住院时间,降低住院费用,提高病人满意度。
[목적]탐토봉합기재복잡주동맥병변병인행경피주동맥강내수복술응용적호리。[방법]입선2011년9월—2013년3월재아과주원적복잡주동맥병변병인공13례,대14처천자부위진행예선매치봉합기술,안조표준방법행조영급복막지가식입,대술후소유병인지혈성공솔、출혈혈종、가성동맥류、감염솔、주원시간、하지심정맥혈전형성、하지제동시간진행통계。[결과]지혈성공솔위100%；술후병발혈종1례,경처리후혈종소퇴,무가성동맥류발생,무술후감염。주원시간9.4 d±3.3 d,무하지심정맥혈전형성,하지제동시간명현축단지4 h~6 h,병인와상인기요배부등불괄감명현감소。[결론]재실시복잡주동맥병변적강내수복술중,여무기타금기증,사용 Perclose Proglide 봉합기예선봉합천자구기술시안전유효적,배이전업적호리가축단병인주원시간,강저주원비용,제고병인만의도。
Objective:To probe into the nursing method of stitching instrument applied to patients with compli-cated aortic disease undergoing percutaneous endovascular aortic repairment.Methods:A total of 13 patients with complicated aortic disease hospitalized in our department from September in 201 1 to March in 2013 in our hospital were selected,14 puncture sites were pre buried suture,and the angiography and stent implantation were implemented according to the standard method.And then to count the success rate of hemostasis,hemor-rhage and hematoma,false aneurysm,infection rate,length of stay,lower limbs deep venous thrombosis,lower limbs braking time in all patients after operation.Results:The success rate of hemostasis was 100%;1 case com-plicated with postoperative hematoma,and the hematoma subsided after treatment,no pseudoaneurysm oc-curred,no postoperative infection occurred.The number of days in hospital was 9.4 d±3.3 d,no lower limbs deep venous thrombosis occurred,lower limbs braking time was shortened to 4 h ~ 6 h,the back discomfort caused by patients lying in bed decreased obviously.Conclusion:The Perclose Proglide suture for pre suturing puncture was safe and effective if there wasn’t other contraindications in the implementation of endovascular re-pairment of patients with complicated aortic disease.It could shorten patients’length of stay combined with the professional nursing,reduce hospitalization expenses and increase patients’satisfaction.
