目的:观察腰椎间盘突出症(lumbar disc herniation, LDH)诱导疼痛大鼠脊髓中 CXC 趋化因子配体1[chemokine(C-X-C motif) ligand 1,CXCL1] mRNA的表达变化。方法:将64只成年雄性SD大鼠随机分为LDH模型组(LDH组,32只)和假手术组(Sham组,32只)。LDH组在左L5神经根近背根神经节附近自体移植尾部髓核,建立大鼠非压迫性LDH模型,Sham组手术方法同LDH组,但不植入髓核。分别测定两组大鼠术前1 d及术后1、3、5、7、10、14、21 d左后爪机械性缩爪阈值(mechanical withdrawal threshold, MWT)和热缩爪潜伏期(paw withdrawal latency, PWL)。 Sham 及LDH组手术前,术后1、3、5、10 d大鼠各6只取L5节段脊髓,采用实时定量PCR法检测CXCL1 mRNA表达水平。结果:与Sham 组相比,LDH 组MWT在术后1 d即显著降低(P<0.001),持续21 d以上(P<0.001);PWL在术后5 d开始下降(P<0.01),21 d部分恢复正常。与Sham组相比,LDH组大鼠脊髓CXCL1 mRNA表达水平在术后有不同程度的上调(P<0.01或P<0.001)。结论:大鼠脊髓中CXCL1 mRNA的表达上调可能与自体髓核移植所致的神经病理性疼痛的发生有关。

목적:관찰요추간반돌출증(lumbar disc herniation, LDH)유도동통대서척수중 CXC 추화인자배체1[chemokine(C-X-C motif) ligand 1,CXCL1] mRNA적표체변화。방법:장64지성년웅성SD대서수궤분위LDH모형조(LDH조,32지)화가수술조(Sham조,32지)。LDH조재좌L5신경근근배근신경절부근자체이식미부수핵,건립대서비압박성LDH모형,Sham조수술방법동LDH조,단불식입수핵。분별측정량조대서술전1 d급술후1、3、5、7、10、14、21 d좌후조궤계성축조역치(mechanical withdrawal threshold, MWT)화열축조잠복기(paw withdrawal latency, PWL)。 Sham 급LDH조수술전,술후1、3、5、10 d대서각6지취L5절단척수,채용실시정량PCR법검측CXCL1 mRNA표체수평。결과:여Sham 조상비,LDH 조MWT재술후1 d즉현저강저(P<0.001),지속21 d이상(P<0.001);PWL재술후5 d개시하강(P<0.01),21 d부분회복정상。여Sham조상비,LDH조대서척수CXCL1 mRNA표체수평재술후유불동정도적상조(P<0.01혹P<0.001)。결론:대서척수중CXCL1 mRNA적표체상조가능여자체수핵이식소치적신경병이성동통적발생유관。
Objective: To observe the expression of CXCL1 mRNA in the spinal cord of rats with lumbar disc herniation-induced pain. Methods: Sixty-four adult male SD rats were randomly divided into model group(LDH group, n=32) and Sham group (Sham group, n=32). The rats in LDH group were made by implantation of the autologous nucleus pulposus, harvested from the coccygeal vertebra of each tail, on the left L5 nerve root just proximal to the dorsal root ganglion(DRG), which established rat model of noncompressed lumbar intervertebral disc herniation. The rats in Sham group received operation similar to model group except omitting implantation of the autologous nucleus pulposus. Mechanical withdrawal threshold (MWT) and paw withdrawal latency (PWL) of left hind limb were measured at 1 day before the surgery and at days 1, 3, 5, 7, 10, 14 and 21 postoperation, respectively. The spinal cords were collected from Sham group and model group at days 1, 3, 5 and 10 after operation. Then the CXCL1 mRNA expression was detected by Real-time PCR method. Results: Compared with Sham group, the PWT in the model group significantly decreased at day 1th day after operation and maintained for over 21 d. The PWL declined from the 5st day postoperation and partially recovered on the 21st day. Compared with Sham group, the mRNA expression of CXCL1 in the spinal cord of LDH group were upregulated significantly at days 1, 3 and 5, but not at day 10 after surgery. Conclusion: The high expression of CXCL1 in the spinal cord of rats may be associated with the neuropathic pain caused by nucleus pulposus.
