目的：探讨非计划再次手术原因，做好医院非计划再次手术管理，提高医疗安全。方法收集某院近2年35例非计划再次手术案例，对非计划再手术原因进行回顾性分析，同时考察其科室分布及与手术等级的关系。结果非计划再手术发生率为1．08‰，主要在骨科，其发生与手术难度有关，其中术后感染、术后再出血、术后组织坏死、血管危象和植入物松动、脱位等为非计划再手术发生的主要原因。结论要从严落实手术分级准入制和手术安全核查制度，加强术前风险评估和术后监管，加强医务人员专业化培训，减少和避免非计划再次手术的发生。

Objective To analysis the causes for the non-planned reoperation , strengthen the operation quality management and ensure medical safety .Methods To collect the clinical data of 35 non-planned reoperation cases happened in recent 2 years, the causes of them were retrospectively analyzed .The department distribution and surgical grade of above occurrence were also investiga -ted.Results The incidence of non-planned reoperation in the hospital was 1.08‰, mostly distributed in the department of orthope-dics and related to the difficulty of operations .The main reasons for non-planned reoperation were postoperative infection , postoperative bleeding , postoperative tissue necrosis , vascular crisis or implant loosening and dislocation .Conclusion Strictly implement the oper-ation grading access system&operation security verification system , strengthen the preoperative risk assessment and postoperative su-pervision , improve the supervision of operation quality security via strengthening the professional trainings for medical personnel as well as other aspects , reduce and avoid unplanned reoperation occurrence .
