目的:探讨同型半胱氨酸与原发性高血压患者心脏结构的关系。方法纳入我院于2012年1月至2014年2月住院的原发性高血压患者814例及非高血压患者219例,调查其基本病史,并查血常规、肝肾功能、血脂及心脏超声。按是否患有原发性高血压及同型半胱氨酸(Hcy)增高与否分组,比较其心脏超声指标异同。采用线性回归模型分析Hcy与相关因素间关系。结果纳入的高血压患者平均年龄为(70.4±11.9)岁,非高血压患者为(67.0±11.8)岁,其中,高血压患者中男性比例50.6%,非高血压患者男性比例为53.4%。高血压患者的Hcy水平显著高于非高血压患者[(15.41±8.4)μmol/L vs.(14.7±9.2)μmol/L , P<0.001]。高血压患者的左心房内径[(36.08±13.02)mm vs.(34.36±7.7) mm,P<0.001],左心室内径[(29.93±9.7)mm vs.(29.2±6.6) mm,P=0.05],室间隔厚度[(7.41±1.56)mm vs.(7.19±2.23)mm,P=0.023]均显著大于非高血压患者。H型高血压患者左心房内径[(36.2±13.4)mm vs.(34.2±5.9)mm,P=0.085]、左心室内径[(30.1±9.9)mm vs.(28.8±5.7)mm,P=0.210]略大于非H型高血压患者,但差异无统计学意义。血压正常人群中,Hcy增高者左心房内径[(35.1±8.0)mm vs.(32.5±6.6)mm,P=0.017]显著高于Hcy正常患者。线性回归模型分析发现高血压患者Hcy水平与左心房内径(β=0.163,P=0.007)、左心室内径(β=0.123,P=0.006)呈正相关。结论 H 型高血压患者左心房内径明显增大,高血压患者Hcy水平与左心房内径,左心室内径明显相关,Hcy可能与高血压患者心脏结构改变有关。

목적:탐토동형반광안산여원발성고혈압환자심장결구적관계。방법납입아원우2012년1월지2014년2월주원적원발성고혈압환자814례급비고혈압환자219례,조사기기본병사,병사혈상규、간신공능、혈지급심장초성。안시부환유원발성고혈압급동형반광안산(Hcy)증고여부분조,비교기심장초성지표이동。채용선성회귀모형분석Hcy여상관인소간관계。결과납입적고혈압환자평균년령위(70.4±11.9)세,비고혈압환자위(67.0±11.8)세,기중,고혈압환자중남성비례50.6%,비고혈압환자남성비례위53.4%。고혈압환자적Hcy수평현저고우비고혈압환자[(15.41±8.4)μmol/L vs.(14.7±9.2)μmol/L , P<0.001]。고혈압환자적좌심방내경[(36.08±13.02)mm vs.(34.36±7.7) mm,P<0.001],좌심실내경[(29.93±9.7)mm vs.(29.2±6.6) mm,P=0.05],실간격후도[(7.41±1.56)mm vs.(7.19±2.23)mm,P=0.023]균현저대우비고혈압환자。H형고혈압환자좌심방내경[(36.2±13.4)mm vs.(34.2±5.9)mm,P=0.085]、좌심실내경[(30.1±9.9)mm vs.(28.8±5.7)mm,P=0.210]략대우비H형고혈압환자,단차이무통계학의의。혈압정상인군중,Hcy증고자좌심방내경[(35.1±8.0)mm vs.(32.5±6.6)mm,P=0.017]현저고우Hcy정상환자。선성회귀모형분석발현고혈압환자Hcy수평여좌심방내경(β=0.163,P=0.007)、좌심실내경(β=0.123,P=0.006)정정상관。결론 H 형고혈압환자좌심방내경명현증대,고혈압환자Hcy수평여좌심방내경,좌심실내경명현상관,Hcy가능여고혈압환자심장결구개변유관。
Objective To investigate the relationship between homocystine and echocardiography characteristics in Chengdu patients with primary hypertension. Methods Eight hundred and fourteen hypertensive patients and two hundred nineteen normal tensive patients hospitalized in our hospital since January 2012 to February 2014 were included in this study. Homocystine, complete blood test, renal function, liver function and echocardiography were performed. Moreover, patients were divided into two groups according to the homocystine level, or hypertension history. Echocardiography characteristics differences between groups were compared. The relationship between homocystine and echocardiography characteristic were analyzed. Results The homocystine level was higher in hypertensive patients than in normal tensive patients, (15.41±8.4)μmol/L vs. (14.7±9.2)μmol/L, P<0.001. Left atrial diameter, (36.08±13.02)mm vs. (34.36±7.7)mm, P=0.085, and left ventricular diameter, (29.93±9.7)mm vs. (29.2±6.6)mm, P=0.210, in H type hypertensive patients was larger than in non H type hypertensive patients. In normal tensive patients with elevated homocystine level, the left atrial diameter was larger, (35.1±8.0)mm vs. (32.5±6.6)mm(P=0.017) than patients with normal homocystine level. The liner regression model found that homocystine level was related with left atrial diameter (β=0.163, P=0.007) and
<br> left ventricular diameter (β=0.123, P=0.006) in patients with hypertension. Conclusions Left atrial diameter is enlarged in H type hypertensive patients. Homocystine level is related with left atrial diameter and left ventricular diameter in hypertensive patients. Homocystine level may be related with echocardiography changes in hypertensive patients.
