目的:探讨荧光原位杂交(FISH)技术诊断孕早期胚胎停育和自然流产绒毛染色体异常的临床应用价值。方法采用7种探针,应用 FISH 技术针对13、16、18、21、22号和 X、Y 染色体,对60例孕早期胚胎停育和自然流产绒毛组织进行检测分析,同时用30例人工流产绒毛作为对照。结果60例孕早期胚胎停育和自然流产绒毛组织中32例存在染色体数目异常,检出率为53.33%,其中15例16-三体,5例13-三体,4例22-三体,1例21-三体,10例三倍体。30例人工流产绒毛组织中检出染色体数目异常1例,为16-三体,检出率为3.33%,孕早期胚胎停育和自然流产绒毛组织染色异常检出率明显高于人工流产绒毛组织染色体异常,差异有统计学意义(P <0.05)。结论染色体数目异常是导致孕早期胚胎停育和自然流产的主要因素,其中16号染色体三倍体是主要改变,FISH 技术可以快速、简便地检测出流产绒毛组织染色体异常。

목적:탐토형광원위잡교(FISH)기술진단잉조기배태정육화자연유산융모염색체이상적림상응용개치。방법채용7충탐침,응용 FISH 기술침대13、16、18、21、22호화 X、Y 염색체,대60례잉조기배태정육화자연유산융모조직진행검측분석,동시용30례인공유산융모작위대조。결과60례잉조기배태정육화자연유산융모조직중32례존재염색체수목이상,검출솔위53.33%,기중15례16-삼체,5례13-삼체,4례22-삼체,1례21-삼체,10례삼배체。30례인공유산융모조직중검출염색체수목이상1례,위16-삼체,검출솔위3.33%,잉조기배태정육화자연유산융모조직염색이상검출솔명현고우인공유산융모조직염색체이상,차이유통계학의의(P <0.05)。결론염색체수목이상시도치잉조기배태정육화자연유산적주요인소,기중16호염색체삼배체시주요개변,FISH 기술가이쾌속、간편지검측출유산융모조직염색체이상。
Objective To investigate the application value of detection the chromosome abnormalities of chorionic villi tissue of early embryo damage and spontaneous abortion by fluorescent in situ hybridization technology.Methods Seven probes were used to detect 13,16,18,21,22 and X,Y respectively,and six-ty cases of early embryo damage and spontaneous aborted fetuses chorionic villus were analyzed by FISH, and the results were compared with villi tissue from 30 cases of artificial abortion.Results Thirty two of the sixty cases were found chromosomal numerical aberrations,and the detection rate was 53.33%,inclu-ding fifteen cases of 16-trisomy,five cases of 13-trisomy,four cases of 22-trisomy,one case of 21-trisomy and ten cases of triploidy.One of the thirty cases of artificial abortion were found chromosomal numerical aberrations,and the detection rate was 3.33%,which was much lower than that of early embryo damage and spontaneous abortion group.The difference has statistical significance (P <0.05).Conclusion Chro-mosomal numerical aberrations are the major factors for early embryo damage and spontaneous abortion, and the major change occurred in 16-trisomy.FISH analysis is a rapid and easy method in the detection of chromosomal abnormality in chorionic villus.
