目的:探讨骨代谢指标与骨密度变化的相关性,为观察骨质疏松寻找可靠的实验室观察指标。方法测定88例骨关节炎(OA)患者股骨颈、腰椎体和腕部骨密度,同时检测血清中骨代谢指标,包括骨钙素、1,25(OH)2维生素D3、骨源性碱性磷酸酶及抗酒石酸酸性磷酸酶,分析骨量与各骨代谢指标的相关性。结果88例OA患者中,骨密度测定显示骨量正常24例(27.3%),骨量减少45例(51.1%),骨质疏松19例(21.6%);骨钙素与腰椎体( r=-0.417, P =0.001)、股骨颈( r=-0.358,P =0.008)、腕部( r=-0.396,P =0.037)骨密度改变均存在显著负相关性,碱性磷酸酶则与腕部骨密度有负相关性关系( r=-0.304,P =0.012)。酸性磷酸酶和维生素D3与腕部骨密度无相关关系。结论在无骨密度检测设备的情况下,测定骨钙素和碱性磷酸酶升高可提示骨量减少或骨质疏松的存在。

목적:탐토골대사지표여골밀도변화적상관성,위관찰골질소송심조가고적실험실관찰지표。방법측정88례골관절염(OA)환자고골경、요추체화완부골밀도,동시검측혈청중골대사지표,포괄골개소、1,25(OH)2유생소D3、골원성감성린산매급항주석산산성린산매,분석골량여각골대사지표적상관성。결과88례OA환자중,골밀도측정현시골량정상24례(27.3%),골량감소45례(51.1%),골질소송19례(21.6%);골개소여요추체( r=-0.417, P =0.001)、고골경( r=-0.358,P =0.008)、완부( r=-0.396,P =0.037)골밀도개변균존재현저부상관성,감성린산매칙여완부골밀도유부상관성관계( r=-0.304,P =0.012)。산성린산매화유생소D3여완부골밀도무상관관계。결론재무골밀도검측설비적정황하,측정골개소화감성린산매승고가제시골량감소혹골질소송적존재。
Objective To investigate the relationship between the changes of bone mineral density and osseous metabolized in -dexes in order to find reliable laboratory markers for osteoporosis (OP).Methods We recruited 88 patients with osteoarthritis(OA) from our hospital and measured the bone density of femoral neck ,lumbar vertebral body and wrist .At the same time ,serum bone metab-olism indexes including calcitonin,1,25(OH)2 Vitamin D3,bone-specific alkaline phosphatase and tartrate resistant acid phosphatase were examined in these patients .Correlation of the bone metabolism indexes and bone mass was analyzed .Results In the 88 OA pa-tients,there was 24 patients with normal bone mass(27.3%),45 patients with osteopenia(51.1%)and 19 with osteoporosis(21.6%). There was a significant negative correlation between calcitonin and the changes of bone mineral density in lumbar vertebral body ( r=-0.417,P =0.001),femoral neck(r =-0.358,P =0.008),and wrist(r =-0.396,P =0.037).There also was a negative correla-tion between alkaline phosphatase and bone mineral density in wrist ( r =-0.304 ,P =0.012 ) while no correlation between wrist bone density and acid phosphatase or Vitamin D 3 was found.Conclusion In the absence of bone density detection equipment ,determination of serum calcitonin and bone-specific alkaline phosphatase may suggest the presence of osteopenia or osteoporosis .
