目的:分析超声弹性成像在乳腺肿瘤诊断中的价值。方法研究对象为2012年3月~2014年4月在我院就诊的乳腺肿块患者66例,共73个病灶。分别采用超声弹性成像、常规超声检查。所有的患者均给予手术治疗,术中切除组织进行病理检查。结果73个病灶中,良性肿瘤25个,恶性肿瘤48个。超声弹性成像对良性肿瘤的诊断符合率为88.0%、对恶性肿瘤的诊断符合率为91.7%,总的诊断符合率为90.4%;常规超声检查对良性肿瘤的诊断符合率为72.0%、对恶性肿瘤的诊断符合率为75.0%,总的诊断符合率为74.0%。二者比较差异有统计学意义(P <0.05)。恶性肿瘤的应变率比值和面积比(3.29±1.13、1.89±0.67)高于良性肿瘤(2.13±0.97、1.46±0.37),比较差异均有统计学意义( P <0.05)。比值法诊断乳腺恶性肿瘤的灵敏度为65.0%,特异度为83.3%,最佳截点为2.76,ROC 曲线下面积为0.75。面积比法诊断乳腺恶性肿瘤的灵敏度为45.9%,特异度为93.7%,最佳截点为1.97,ROC 曲线下面积为0.71。结论超声弹性成像在乳腺良恶性肿瘤的诊断中有重要价值。

목적:분석초성탄성성상재유선종류진단중적개치。방법연구대상위2012년3월~2014년4월재아원취진적유선종괴환자66례,공73개병조。분별채용초성탄성성상、상규초성검사。소유적환자균급여수술치료,술중절제조직진행병리검사。결과73개병조중,량성종류25개,악성종류48개。초성탄성성상대량성종류적진단부합솔위88.0%、대악성종류적진단부합솔위91.7%,총적진단부합솔위90.4%;상규초성검사대량성종류적진단부합솔위72.0%、대악성종류적진단부합솔위75.0%,총적진단부합솔위74.0%。이자비교차이유통계학의의(P <0.05)。악성종류적응변솔비치화면적비(3.29±1.13、1.89±0.67)고우량성종류(2.13±0.97、1.46±0.37),비교차이균유통계학의의( P <0.05)。비치법진단유선악성종류적령민도위65.0%,특이도위83.3%,최가절점위2.76,ROC 곡선하면적위0.75。면적비법진단유선악성종류적령민도위45.9%,특이도위93.7%,최가절점위1.97,ROC 곡선하면적위0.71。결론초성탄성성상재유선량악성종류적진단중유중요개치。
Objective:To analyse the value of ultrasound elastography in the diagnosis of breast tumor. Methods The re-search object was 66 patients with breast lumps in our hospital,from March 2012 to April 2014 ,a total of 73 lesions. Ultra-sound elastography and conventional ultrasound were used. All patients were performed surgical treatment,intraoperative removal of tissue for pathologic examination. Results Among the 73 lesions,25 were benign and 48 were malignant. The di-agnostic accordance rate of ultrasound elasticity imaging of benign tumor was 88. 0% ,the coincidence rate to the diagnosis of malignant tumor was 91. 7% ,the total diagnostic coincidence rate was 90. 4% ;Conventional ultrasound diagnosis coin-cidence rate of benign tumor was 72. 0% ,the coincidence rate to the diagnosis of malignant tumor was 75. 0% ,the total diagnostic coincidence rate was 74. 0% . The comparative difference was statistically significant(P < 0. 05). The strain rate ratio and area ratio(3. 29 ± 1. 13 ,1. 13 ± 0. 67)of the Malignant tumor were higher than that of benign tumor(2. 13 ± 0. 97,0. 97 ± 0. 37),and the comparative differences were statistically significant(P < 0. 05). By Ratio method,the sensitivity and specificity of breast malignant tumorˊs diagnosis was 65. 0% and 83. 3% ,and the best cutoff point is 2. 76, the area under the ROC curve was 0. 75. By Area ratio method ,the sensitivity and specificity of breast malignant tumor’ s diagnosis was 45. 9% and 93. 7% ,and the best cutoff point is 1. 97,the area under the ROC curve was 0. 71. Conclu-sion Ultrasound elastography has important value in the diagnosis of benign and malignant breast tumors.
