目的：了解北京市H区农村医患信任现状及基层卫生服务内容变化对医患信任的影响。方法：采用目的抽样的方法，对北京市H区3个乡镇的12位村干部、12位村医以及24位村民进行实地观察和访谈，对访谈结果进行定性分析。结果：乡土社会“差序格局”背景之下的村落医患信任，以人际信任为主体，呈现总体信任、个别不满的局面。结论：从计划经济到市场经济过渡的过程中，由于村民健康需求提高、可选择性增强以及村卫生室服务重点转移、政策落实不到位，导致基层卫生服务内容的变化，对村落医患信任产生一定的影响。
목적：료해북경시H구농촌의환신임현상급기층위생복무내용변화대의환신임적영향。방법：채용목적추양적방법，대북경시H구3개향진적12위촌간부、12위촌의이급24위촌민진행실지관찰화방담，대방담결과진행정성분석。결과：향토사회“차서격국”배경지하적촌락의환신임，이인제신임위주체，정현총체신임、개별불만적국면。결론：종계화경제도시장경제과도적과정중，유우촌민건강수구제고、가선택성증강이급촌위생실복무중점전이、정책락실불도위，도치기층위생복무내용적변화，대촌락의환신임산생일정적영향。
Objective:To understand the physician-patient the trust in H District in rural Beijing and discuss how it had been influenced by the changes of rural health services. Methods:This paper is based on observations and interviews on 12 village cadres, 12 village doctors and 24 villagers. We carried out qualitative analysis on the result. Results:In a traditional Chinese society which is characterized by the"grade structure", the basis for the trust between physician and patient is interpersonal trust. Most patients can trust their doctors. Conclusion:In the process of transition from the planned economy to the market economy, there are various factors contributing to the physician-patient trust in rural areas. Firstly, rural patients ask for better health care. Secondly, they get more choices on health services. Thirdly, the focus of service of village clinic is changed. Last but not the least, policy can't be carried completely out.
