目的:研究原发性高血压合并糖尿病患者心血管自主神经功能与血压形态的关系。方法:入选原发性高血压患者70例,其中合并糖尿病者35例为试验组(EH-DM组),血糖正常者35例为对照组(EH组)。两组患者均行标准心血管自主神经功能检查以及24 h动态血压监测。结果:EH-DM组自主神经功能评分显著高于EH组(P<0.05),昼夜收缩压下降率(ΔSBP%)显著低于EH组(P<0.05),具有更高的非杓型血压形态比例(P<0.05)。其中,昼夜收缩压下降率与自主神经病变评分呈负相关(r=-0.328,P<0.05)。结论:合并糖尿病的高血压病患者,心血管自主神经功能评分高,非杓型血压形态比例高,提示血糖可能通过损害心血管自主神经功能改变血压形态。
目的:研究原髮性高血壓閤併糖尿病患者心血管自主神經功能與血壓形態的關繫。方法:入選原髮性高血壓患者70例,其中閤併糖尿病者35例為試驗組(EH-DM組),血糖正常者35例為對照組(EH組)。兩組患者均行標準心血管自主神經功能檢查以及24 h動態血壓鑑測。結果:EH-DM組自主神經功能評分顯著高于EH組(P<0.05),晝夜收縮壓下降率(ΔSBP%)顯著低于EH組(P<0.05),具有更高的非杓型血壓形態比例(P<0.05)。其中,晝夜收縮壓下降率與自主神經病變評分呈負相關(r=-0.328,P<0.05)。結論:閤併糖尿病的高血壓病患者,心血管自主神經功能評分高,非杓型血壓形態比例高,提示血糖可能通過損害心血管自主神經功能改變血壓形態。
목적:연구원발성고혈압합병당뇨병환자심혈관자주신경공능여혈압형태적관계。방법:입선원발성고혈압환자70례,기중합병당뇨병자35례위시험조(EH-DM조),혈당정상자35례위대조조(EH조)。량조환자균행표준심혈관자주신경공능검사이급24 h동태혈압감측。결과:EH-DM조자주신경공능평분현저고우EH조(P<0.05),주야수축압하강솔(ΔSBP%)현저저우EH조(P<0.05),구유경고적비표형혈압형태비례(P<0.05)。기중,주야수축압하강솔여자주신경병변평분정부상관(r=-0.328,P<0.05)。결론:합병당뇨병적고혈압병환자,심혈관자주신경공능평분고,비표형혈압형태비례고,제시혈당가능통과손해심혈관자주신경공능개변혈압형태。
Objective:To explore the relationship between cardiac autonomic function and blood pressure circadian rhythm in diabetic hypertensive patients.Method:70 essential hypertensive patients were recruited, 35 complicate with diabetes as the experimental group (group EH-DM) and the other 35 with normal blood glucose as the control group (group EH).They underwent standard cardiovascular autonomic function tests (SCAFT) and ambulatory blood pressure monitoring(ABPM).Result:The SCAFT score was higher in the group EH-DM than in the group EH (P<0.05).The decline of SBP at night(ΔSBP%)was lower in the group EH-DM than in the group EH(P<0.05).The ratio of non-dipping circadian rhythm in the group EH-DM was higher than that in the group EH (P<0.05).The decline of SBP at night and SCAFT score appeared in an obvious negative correlation(r=-0.328,P<0.05).Conclusion:The diabetic hypertensive patients have higher SCAFT score and higher ratio of non-dipping circadian rhythm.Blood glucose may affect blood pressure circadian rhythm through impairing cardiovascular autonomic function.
