目的:研究用传统方法酸溶血试验、蔗糖溶血试验以及流式细胞术检测红细胞、白细胞上的CD55、CD59对PNH患者外周血检测结果,了解传统方法与流式细胞术检测方法的敏感性和特异性以及对PNH患者诊断的临床应用。方法:用酸溶血试验、蔗糖溶血试验和流式细胞术分别对40例PNH患者和30例健康对照组进行检测,并对三种检测结果进行统计学分析。结果:流式细胞术检测红细胞、白细胞上的CD55、CD59阳性率和蔗糖溶血试验无明显差异(P>0.05),明显高于酸溶血试验(P<0.05),酸溶血试验阳性率明显低于蔗糖溶血试验P<0.05,三种方法分别和对照组相比,P均<0.05,说明三种方法检测PNH组阳性率均明显高于对照组。结论:流式细胞术检测红细胞、白细胞上的CD55、CD59诊断PNH与传统的酸溶血试验和蔗糖溶血试验相比,不仅特异性好,敏感性也好,但技术要求非常高,仪器和试剂成本也较贵,比较适合大医院开展;传统方法:酸溶血试验特异性高敏感性低,蔗糖溶血试验特异性低敏感性高,但两者费用低廉,容易操作,109联合应用有助于提高PNH患者的诊断,所以非常适合基层医院开展应用。

Objective Research in the traditional method of acid hemolysis test,sucrose hemolysis test and flow cytometry detection of red blood cells,white blood cells of CD55, CD59 on peripheral blood in patients with PNH test results. Methods With acid hemolysis test,sucrose hemolysis test and flow cytometry respectively in 40 patients with PNH patients and 30 healthy controls were detected,and the three test results for statistical analysis.Results Flow cytometry detection of red blood cells,white blood cells of CD55,CD59 on no difference between the positive rate and sucrose hemolytic test, significantly higher than acid hemolysis test,P> 0.05,P < 0.05, acid hemolysis test were significantly lower than sucrose hemolytic test,P< 0.05,three methods were compared with control group,P< 0.05,suggesting that positive rate of three methods to detect PNH group were significantly higher than the control group.ConclusionsFlow cytometry measure red blood cells,white blood cells of CD55, CD59 on the diagnosis of PNH with traditional acid compared hemolysis test and sucrose hemolytic test, not only good specificity,sensitivity.
