目的：观察和分析2型糖尿病（DM）合并糖尿病肾病（DN）患者血清 IL-2、IL-8水平的变化及其临床意义。方法选取2型 DM 患者120例作为研究对象，根据其尿白蛋白排泄率（UAER）水平将其分为 DM 组（UAER＜20μg／min，共纳入42例）、DN1组（20μg／min≤UAER≤200μg／min，共纳入37例）和 DN2组（UAER＞200μg／min，共纳入41例），选取同期体检结果正常的健康人40例作为对照组。对四组研究对象的血清 IL-2水平和血清 IL-8水平进行检测和比较。结果对照组研究对象的血清 IL-2水平显著高于 DM 组（P ＜0．05），DM 组患者的血清 IL-2水平显著高于 DN1组（P ＜0．05），DN1组患者的血清 IL-2水平显著高于 DN2组（P ＜0．05），对照组研究对象的血清 IL-8水平显著低于 DM 组（P ＜0．05），DM 组患者的血清 IL-8水平显著低于 DN1组（P ＜0．05）；DN 患者的血清 IL-2水平与 UAER 呈负相关关系（r＝－0．683，P ＜0．05），而血清 IL-8与 UAER 呈正相关关系（r＝0．715，P ＜0．05）。结论血清 IL-2水平的下降和血清 IL-8的升高可提示 DN 的发病，且此两个指标与 DN 患者的尿蛋白量具有良好的相关性，可作为 DN 早期诊断和病情预测的辅助指标。

목적：관찰화분석2형당뇨병（DM）합병당뇨병신병（DN）환자혈청 IL-2、IL-8수평적변화급기림상의의。방법선취2형 DM 환자120례작위연구대상，근거기뇨백단백배설솔（UAER）수평장기분위 DM 조（UAER＜20μg／min，공납입42례）、DN1조（20μg／min≤UAER≤200μg／min，공납입37례）화 DN2조（UAER＞200μg／min，공납입41례），선취동기체검결과정상적건강인40례작위대조조。대사조연구대상적혈청 IL-2수평화혈청 IL-8수평진행검측화비교。결과대조조연구대상적혈청 IL-2수평현저고우 DM 조（P ＜0．05），DM 조환자적혈청 IL-2수평현저고우 DN1조（P ＜0．05），DN1조환자적혈청 IL-2수평현저고우 DN2조（P ＜0．05），대조조연구대상적혈청 IL-8수평현저저우 DM 조（P ＜0．05），DM 조환자적혈청 IL-8수평현저저우 DN1조（P ＜0．05）；DN 환자적혈청 IL-2수평여 UAER 정부상관관계（r＝－0．683，P ＜0．05），이혈청 IL-8여 UAER 정정상관관계（r＝0．715，P ＜0．05）。결론혈청 IL-2수평적하강화혈청 IL-8적승고가제시 DN 적발병，차차량개지표여 DN 환자적뇨단백량구유량호적상관성，가작위 DN 조기진단화병정예측적보조지표。
Objective To observe and analyze the changes and the clinical significances of the serum interleukin-2 (IL-2)level and the serum interleukin-8 (IL-8)level of the patients with type 2 diabetes mellitus (DM)complicated with diabetes nephropathy (DN).Methods 120 patients with Type 2 DM were selected as the research objects and di-vided into DM group (UAER<20 μg/min,42 cases included),DN1 group (20 μg/min≤UAER≤200 μg/min,37 ca-ses included)and DN2 group (UAER>200 μg/min,41 cases included)according to their urinary albumin excretion rate (UAER)levels.40 cases of healthy people with normal examination results were selected as the control group. The serum IL-2 level and the serum IL-8 level of the objects in the four groups were detected and compared.Results The serum IL-2 level of the objects in the control group was higher than that in DM group (P <0.05),the serum IL-2 level of the patients in the DM group was significantly higher than that in DN1 group (P <0.05),the serum IL-2 level of the patients in DN1 group was significantly higher than that in DN2 group (P <0.05).The serum IL-8 level of the objects in the control group was lower than that in DM group (P <0.05),the serum IL-8 level of the patients in the DM group was significantly lower than that in DN1 group (P <0.05),the serum IL-8 level of the patients in DN1 group was significantly lower than that in DN2 group (P <0.05).There was a negative correlation between the serum IL-2 level and UAER of the patients with DN (r=-0.683,P <0.05),and the serum IL-8 level is positively correlated with UAER (r=0.715,P <0.05).Conclusion The decrease of the serum IL-2 level and the increase of the serum IL-8 may be indicative of the incidence of DN,and there are good correlations between such two indexes and the urinary protein level of the patients with DN.The indexes can be used as auxiliary indicators for early diagnosis and prognosis of DN.
