Anti-cyclic citrullinated peptide antibody%Rheumatoid factor%Rheumatoid arthritis
目的:探讨抗环瓜氨酸肽(抗-CCP)抗体和类风湿因子(RF)在类风湿关节炎(RA)中的诊断价值。方法分别检测抗 CCP 抗体(酶联免疫吸附实验)、RF 及 CRP(免疫比浊法)、ESR(魏式法)在 RA 组、非 RA 组和健康人中的表达并比较分析。结果RA 组和非 RA 组 CRP、ESR 表达水平无显著性差异(P >0.05),而 RA 组 RF 表达显著高于非 RA 组,两者有显著性差异(P <0.01);RA 组抗 CCP 抗体和 RF 阳性率明显高于非 RA 组和对照组,数据经统计学比较差异有统计学意义(P <0.01)。抗 CCP 抗体的敏感性为72.72%,特异性为92.68%;RF 的敏感性为86.36%,特异性为85.37%;抗 CCP 抗体和 RF 联合检测敏感性为63.64%,特异性为95.12%。结论在 RA 诊断中,抗-CCP 有较高的特异性,RF 有较高的敏感性,两者联合检测有利于早期诊断 RA。

목적:탐토항배과안산태(항-CCP)항체화류풍습인자(RF)재류풍습관절염(RA)중적진단개치。방법분별검측항 CCP 항체(매련면역흡부실험)、RF 급 CRP(면역비탁법)、ESR(위식법)재 RA 조、비 RA 조화건강인중적표체병비교분석。결과RA 조화비 RA 조 CRP、ESR 표체수평무현저성차이(P >0.05),이 RA 조 RF 표체현저고우비 RA 조,량자유현저성차이(P <0.01);RA 조항 CCP 항체화 RF 양성솔명현고우비 RA 조화대조조,수거경통계학비교차이유통계학의의(P <0.01)。항 CCP 항체적민감성위72.72%,특이성위92.68%;RF 적민감성위86.36%,특이성위85.37%;항 CCP 항체화 RF 연합검측민감성위63.64%,특이성위95.12%。결론재 RA 진단중,항-CCP 유교고적특이성,RF 유교고적민감성,량자연합검측유리우조기진단 RA。
Objective To explore the diagnostic value of anti-cyclic citrullinated peptide (anti-CCP)antibody and rheumatoid factor (RF)in rheumatoid arthritis.Methods Enzyme-linked immunosorbent assay (ELISA)and immuno-turbidimetric assay and Westergrem assay were employed to analyze the expressing levels of anti-CCP antibody,RF, CRP and ESR for RA,non-RA and healthy control groups respectively.Results the expressing levels of CRP and ESR of RA and non-RA groups were not statistically significant (P>0.05),while the expressing level of RF of RA group was much higher than that of the non-RA group,and the difference was statistically significant (P <0.01);the positive rate of anti-CCP antibody and RF for RA group was higher than that of the non-RA and control groups,and the differ-ence was statistically significant (P <0.01).The sensitivity of anti-CCP antibody and RF were 72.72% and 86.36%, respectively;the specificity of anti-CCP antibody and RF were 92.68% and 85.37%,respectively.The joint-detection sensitivity and specificity for anti-CCP antibody and RF were 63.64% and 95.12,respectively.Conclusion In the diag-nosis of RA,anti-CCP detection is highly specific,RF is highly sensitive,joint detection of anti-CCP and RF is conducive for the early diagnosis of RA.
