目的探讨超声成像技术用于气管导管插管定位的可行行及准确性。方法择期行腹腔镜胆囊切除术的患者30例,年龄20~60岁,ASA分级I~I 级。静脉麻醉诱导后插入气管导管,同时采取呼吸末二氧化碳监测和超声监测下胸膜滑动征的出现确定导管位置,最后用纤支镜确认。结果超声观察胸膜滑动征可以确定气管导管的位置并且扫查所用时间较呼气末二氧化碳监测短,同时有效区分支气管插管。结论用超声观察胸膜滑动征确定和调整ETT的位置具有可行性,且判断迅速并且及时排除支气管插管。

목적탐토초성성상기술용우기관도관삽관정위적가행행급준학성。방법택기행복강경담낭절제술적환자30례,년령20~60세,ASA분급I~I 급。정맥마취유도후삽입기관도관,동시채취호흡말이양화탄감측화초성감측하흉막활동정적출현학정도관위치,최후용섬지경학인。결과초성관찰흉막활동정가이학정기관도관적위치병차소사소용시간교호기말이양화탄감측단,동시유효구분지기관삽관。결론용초성관찰흉막활동정학정화조정ETT적위치구유가행성,차판단신속병차급시배제지기관삽관。
Objective To explore the feasibility of endotracheal intubation and positioning accuracy of ultrasonic imaging technology. Methods 30 patients undergoing elective laparoscopic cholecystectomy patients, age 20~60 years old, ASA grade I~I . Vein after induction of anesthesia, to insert the endotracheal tube, at the same time take ETCO2 monitoring and ultrasound monitoring under the pleura sliding signs to determine the position of the catheter, and final y confirmed by fiberoptic bronchoscopy. Results The ultrasound observation of pleura sliding signs can determine the endotracheal tube position and scan the end expiratory carbon dioxide monitoring time is short, and distinguish the bronchial intubation. Conclusion Ultrasound observation of pleura sliding signs to determine and adjust the position of ETT is feasible, and quick to judge and remove bronchial intubation.
