当今很多危急重症患者为了挽救生命都需要进行气道切开来维持呼吸道的通畅稳定，由于短时间不能立即封管，给患者造成了一系列的功能障碍。如吞咽、言语、咳嗽、呼吸道正常气压气流、肺顺应性降低、肺部感染等。说话瓣膜是一个单向通气阀门，衔接在气管套管处：吸气时瓣膜打开，气流通过瓣膜打开的缺口进入气道：呼气时瓣膜关闭，气流从气管导管与气管间隙通过上呼吸道呼出，能有效地改善吞咽、言语功能减少误吸，增加社交等。但还没有人说对话瓣膜能否改善促进呼吸功能进行研究探讨。本文就气切后患者功能障碍、说话瓣膜的应用方法以及佩戴说话瓣膜带来的好处进行简单归纳总结。说明佩戴说话瓣膜对改善呼吸功能、提高血氧饱和度、减少误吸带来的肺部炎症发生、缩短拔管时间等有明显效果，值得进一步探讨推广。

Nowadays, Many critical y il patients need to do the tracheotomy in order to keep respiratory tract unobstructed for saving their lives.However, the airway could not be sealed in a short time, and it lead patients to a series of dysfunction,such as the dysphagia,the aphasia,the cough,the regular pressure and airflow of the airway, the lung compliance,the high rate of the lung infection and etc. The speaking value which links up to the tracheal tube is a one-way hose breather.The speaking value opens when we inspired air and the air gets into the airway from the entrance of the speaking value .In contrast,it closes when we breath and the air flows from the gap of the trachea tube and the the trachea through the upper airway.It can availably improve the swal ow,the language,the social intercourse and decrease the aspiration.However,there is no more paper discussing the function of the speaking value in the respiratory function. In this paper, we made a simple generalization about the application methods of the speaking value and the benefit of it after tracheotomy.We found that the speaking value improved the respiratory function ,the SPO2,decreased the rate of lung infection resulting from the dysphagia and shortened the time of sealing the airway. It worths further discussing.
