目的:探讨声辐射脉冲成像技术(ARFI)在定量诊断弥漫性脂肪肝和评价水飞蓟宾磷脂复合物疗效的临床应用价值。方法选取2012年1月至2014年1月医院就诊的弥漫性脂肪肝患者50例,分为轻度、中度和重度3组。同时选取同期体检的50例健康人作为对照组。对比四组的声触诊组织定量(VTQ)值、肝功能和血脂水平,分析 VTQ 值和肝功能、血脂水平之间的相关性。采用水飞蓟宾磷脂复合物治疗4周后,对比所有患者治疗前后的 VTQ、肝功能和血脂水平。结果随着脂肪肝程度加重,VTQ 值逐渐升高( P ﹤0.05),丙氨酸氨基转移酶(ALT)、天门冬酸氨基转移酶(AST)、三酰甘油(TG)和总胆固醇(TC)水平也逐渐升高( P ﹤0.05)。采用 Spearman 相关分析,VTQ 值和 ALT( r =0.405,P =0.042 ﹤0.05)、AST( r =0.519,P =0.044 ﹤0.05)、TG( r =0.490,P =0.038 ﹤0.05)、TC( r =0.523, P =0.037 ﹤0.05)之间均存在正相关。3组患者治疗4周后的 VTQ,ALT,AST,TG 和 TC 水平均显著低于治疗前( P ﹤0.05)。结论 ARFI可用于量化诊断弥漫性脂肪肝和评价水飞蓟宾磷脂复合物的疗效。

목적:탐토성복사맥충성상기술(ARFI)재정량진단미만성지방간화평개수비계빈린지복합물료효적림상응용개치。방법선취2012년1월지2014년1월의원취진적미만성지방간환자50례,분위경도、중도화중도3조。동시선취동기체검적50례건강인작위대조조。대비사조적성촉진조직정량(VTQ)치、간공능화혈지수평,분석 VTQ 치화간공능、혈지수평지간적상관성。채용수비계빈린지복합물치료4주후,대비소유환자치료전후적 VTQ、간공능화혈지수평。결과수착지방간정도가중,VTQ 치축점승고( P ﹤0.05),병안산안기전이매(ALT)、천문동산안기전이매(AST)、삼선감유(TG)화총담고순(TC)수평야축점승고( P ﹤0.05)。채용 Spearman 상관분석,VTQ 치화 ALT( r =0.405,P =0.042 ﹤0.05)、AST( r =0.519,P =0.044 ﹤0.05)、TG( r =0.490,P =0.038 ﹤0.05)、TC( r =0.523, P =0.037 ﹤0.05)지간균존재정상관。3조환자치료4주후적 VTQ,ALT,AST,TG 화 TC 수평균현저저우치료전( P ﹤0.05)。결론 ARFI가용우양화진단미만성지방간화평개수비계빈린지복합물적료효。
Objective To investigate the clinical application value of acoustic radiation force impulse techniques (ARFI) in quantitative diagnosis of diffuse fatty liver and to evaluate the effect of silybin phospholipid complex therapy. Methods 50 patients with diffuse fat-ty liver in our hospital from January 2012 to January 2014 were selected and divided into mild, moderate and severe groups. Meanwhile 50 people of healthy physical examination over the same period were selected as the control group. The virtual touch tissue quantication (VTQ) value, liver function and blood lipid levels were compared among the patients in the four groups. The correlation between the VTQ value with the liver function and lipid levels was analyzed. After 4 - week silybin phospholipid complex treatment, the VTQ values, liver function and blood lipid levels were compared between before and after treatment in all the patients. Results The VTQ value was increased with the gradual increase of fatty liver severity( P ﹤ 0. 05) . The ALT, AST, TG and TC levels were gradually increased with the increase of fatty liver degree( P ﹤ 0. 05) . The Spearman correlation analysis showed that the VTQ value was positively correlated with ALT( r = 0. 405, P = 0. 042 ﹤ 0. 05), AST( r = 0. 519, P = 0. 044 ﹤ 0. 05), TG( r = 0. 490, P = 0. 038 ﹤ 0. 05) and TC( r = 0. 523, P = 0. 037 ﹤ 0. 05). The VTQ, ALT, AST, TG and TC levels after 4 - week treatment were significantly lower than those before treatment ( P ﹤ 0. 05). Conclusion The ARFI technique can be used for quantifying diffuse fatty liver and evaluating the curative effect of sily-bin phospholipid complex.
