目的 评价对脐带血清对持续性角膜上皮缺损的治疗效果和安全性.方法 常规治疗至少2周后无好转的持续性角膜上皮缺损11例(16眼),应用脐带血清滴眼6次/d.用药后对患者的症状(疼痛、畏光、流泪)、体征(睫状充血或结膜充血,角膜上皮缺损的区域面积)进行记录,2周内上皮缺损完全愈合者评定为有效,2～4周愈合者评定为部分有效,超过4周或采用其它方法治疗者评定为无效.结果 上皮缺损脐带血清治疗前平均病程为(68.56±41.97)d,上皮缺损面积为(8.74±7.23) mm2.脐带血清治疗有效12眼,部分有效3眼,无效1眼,然而,上皮缺损无效的1眼在用药7周后治愈.有效眼和部分有效眼的平均愈合时间为(11.73 ±6.96)d.治疗及随访过程中未出现感染等并发症.结论 应用脐带血清滴眼治疗持续性角膜上皮缺损是安全和有效的.

Objective To evaluate the therapeutic effect and safety of umbilical cord serum for persistent corneal epithelial defects.Methods Sixteen eyes of 11 patients with persistent epithelial defect had persisted for at least two weeks,and conventional treatment had failed for them.All patients were treated with umbilical cord serum eyedrops 6 times a day.The symptoms and signs after treatment were recorded.Treatment was identitied as effective if epithelial defect was healing in 2 weeks,as partially effective if healing within 2 to 4 weeks,and as ineffective if healing time was more than 1 month or patients need adjuvant therapy.There was no complication of corneal infection during therapy and follow-up periods.Results Mean duration of epithelial defect before treatment was (68.56 ± 41.97) days,and mean area was (6.68 ± 5.23) mm2.Umbilical cord serum therapy was effective in 12 eyes,partially effective in 3 eyes,and ineffective in 1 eye.Nevertheless,the epithelial defects in the ineffective eye were eventually healed within 7 weeks.Mean healing time in effective and partially effective cases was (11.73 ± 6.96) days.No complication of corneal infection was observed during treatment and follow-up period.Conclusion Umbilical cord serum eyedrops for the treatment of persistent epithelial defect is safe and effective.
