目的评价角膜基质层间羊膜移植治疗大泡性角膜病变的临床效果.方法大泡性角膜病变患者10例(10眼).其中男6例,女4例,年龄45～78岁,平均(65.0±3.30)岁.白内障超声乳化加人工晶体植入术后7例,青光眼白内障联合术后3例.术中用板层刀作一带蒂的角膜瓣,植入4 mm×5 mm羊膜一片;以10-0线间断缝合角膜瓣3针,并对其进行近期和远期的疗效观察.结果羊膜移植术后随访3～6月,角膜上皮10 d内均完全修复,8例疼痛完全消失,2例周边角膜局限小泡,4例术后视力提高.结论角膜基质层间羊膜移植不仅可以减轻疼痛而且可部分恢复视力,对暂不能获得有活性角膜材料以及不适应角膜移植手术的大泡性角膜病变患者,角膜基质层间羊膜移植仍有其重要的临床意义.
목적 평개각막기질층간양막이식치료대포성각막병변적림상효과.방법 대포성각막병변환자10례(10안).기중남6례,녀4례,년령45～78세,평균(65.0±3.30)세.백내장초성유화가인공정체식입술후7례,청광안백내장연합술후3례.술중용판층도작일대체적각막판,식입4 mm×5 mm양막일편;이10-0선간단봉합각막판3침,병대기진행근기화원기적료효관찰.결과 양막이식술후수방3～6월,각막상피10 d내균완전수복,8례동통완전소실,2례주변각막국한소포,4례술후시력제고.결론 각막기질층간양막이식불부가이감경동통이차가부분회복시력,대잠불능획득유활성각막재료이급불괄응각막이식수술적대포성각막병변환자,각막기질층간양막이식잉유기중요적림상의의.
Objective To evaluate the therapeutic effects of corneal stroma amniotic membrane transplantation for bullous kerafopthy.Methods Ten eyes of 10 patients with bullous keratopathy were included (6 males and 4 females).The average age was 65.0 ± 3.30 years old (ranged from 45 to 78 years old).7 eyes were after phacoemulsification and intraocular lens implantation.Other 3 eyes were after glaucoma and cataract surgery.Pedicle corneal flap was made by microkeratome.Amniotic membrane of 4 mm × 5 mm was implanted.Then the flap was sutured 3 stitches with 10-0 nylon sutureo.The postoperative effects of short and long term were observed and recorded.Results All the patients were followed up for 3 to 6 months.Corneal epithelium of all eyes were recovered within 10 days.8 eyes with eye pain were completely healed.Limitated vesicles in peripheral cornea occurred in 2 eyes.The postoperative visual acuity were improved significantly in 4 patients.Conclusion Corneal stroma amniotic membrane transplantation can not only relieve pain,but also improve visual acuity.Thus,it is an important clinical temporary treatment for bullous keratopathy when donor cornealtissues were limited and restricted.
