目的:分析慢性阻塞性肺疾病(COPD )并发肺动脉高压患者的临床特征,探讨胸部CT及肺功能对肺动脉高压的诊断价值。方法回顾性分析COPD患者72例,经超声心动图测量肺动脉收缩压(PASP ),根据PASP结果分为对照组、轻度肺动脉高压组、中重度肺动脉高压组,比较三组患者红细胞压积、血红蛋白、D_II聚体、动脉血气分析、肺功能、CT图像上的主肺动脉直径与主动脉直径等测值。结果中重度肺动脉高压组PaCO2显著高于对照组及轻度肺动脉高压组,差异有统计学意义( P<0.05)。相比对照组,轻度及中重度肺动脉高压组的主肺动脉直径、主肺动脉直径/降主动脉直径比值均大于对照组,差异有统计学意义( P<0.01),用力肺活量(FVC )、一秒用力呼气容积(FEV1)、FEV1/FVC、一氧化碳弥散量差异均无统计学意义( P>0.05)。结论 CT中的主肺动脉直径及主肺动脉与降主动脉直径比值可作为筛查肺动脉高压的指标。

목적:분석만성조새성폐질병(COPD )병발폐동맥고압환자적림상특정,탐토흉부CT급폐공능대폐동맥고압적진단개치。방법회고성분석COPD환자72례,경초성심동도측량폐동맥수축압(PASP ),근거PASP결과분위대조조、경도폐동맥고압조、중중도폐동맥고압조,비교삼조환자홍세포압적、혈홍단백、D_II취체、동맥혈기분석、폐공능、CT도상상적주폐동맥직경여주동맥직경등측치。결과중중도폐동맥고압조PaCO2현저고우대조조급경도폐동맥고압조,차이유통계학의의( P<0.05)。상비대조조,경도급중중도폐동맥고압조적주폐동맥직경、주폐동맥직경/강주동맥직경비치균대우대조조,차이유통계학의의( P<0.01),용력폐활량(FVC )、일초용력호기용적(FEV1)、FEV1/FVC、일양화탄미산량차이균무통계학의의( P>0.05)。결론 CT중적주폐동맥직경급주폐동맥여강주동맥직경비치가작위사사폐동맥고압적지표。
Objective To investigate the clinical characteristics of patients with chronic obstructive pulmonary disease (COPD) associ-ated with pulmonary hypertension(PH) ,and evaluate the diagnostic value of pulmonary function test and chest computed tomography for predicting pulmonary hypertension.Methods Retrospective study of 72 patients with COPD ,all patients were divided into three groups according to pulmonary artery systolic pressure (PASP) measured by echocardiography. Hematokrit ,hemoglobin ,D_Dimer , blood_gas analysis ,pulmonary function test ,diameters of main pulmonary artery (MPAD) and aorta were compared among the groups.Results The PaCO2 in moderate_severe PH group was significantly higher than that of control group and mild PH group ,there were statistically significant differences ( P<0.05 ) .Compared with control group ,the MPAD and the ratio of main pulmonary artery di-ameter to descending aorta diameter were significantly lager in mild PH group and moderate_severe PH group ,there were statistically significant differences ( P<0.01 ) ,while FVC ,FEV1 ,FEV1/FVC ,DLCO showed no significant differences ( P>0.05 ).Conclu-sions The main pulmonary artery diameter and the ratio of main pulmonary artery diameter to descending aorta diameter on CT scan can be used to screen for PH.
