目的:探讨原发性高血压患者动态脉压(PP )与心率减速力(DC )的关系。方法对原发性高血压患者180例进行动态血压监测,按动态PP分为24h平均PP<60mmHg组(PP1组,n=90)、24h平均PP≥60mmHg组(PP2组,n=90),正常对照组(n=60)。所有病例均接受动态心电图检查,离线分析计算DC值,比较分析各组PP和DC值。结果原发性高血压患者PP值水平较对照组升高,DC值水平较对照组降低,差异均有统计学意义( P<0.01),且DC值随PP增高而有进一步降低趋势( P<0.01)。Spearman相关分析显示动态PP与DC呈负相关( r=-0.68,P<0.05)。结论原发性高血压患者存在自主神经功能受损,且随着动态PP的增大自主神经功能受损越严重。

Objective To explore the relationship between ambulatory pulse pressure (PP) and deceleration capacity of rate(DC) in patients with essential hypertension.Methods 180 patients with essential hypertension were given ambulatory blood pressure monitor-ing ,they were assigned randomly into 2 groups according to the levels of ambulatory PP :average PP of 24 hours ≥60mmHg group (group PP1 ,n=90) ,average PP of 24 hours ≥ 60 mmHg group (group PP2 ,n=90) ,and normal control group (n=60).All of them were given dynamic electrocardiographic examination ,then calculated and analyzed DC value off -line ,PP and DC value of each group were compared and analyzed .Results PP in patients with essential hypertension were raised by contrast to that in control group ,DC in patients with essential hypertension were reduced ,the differences had statistically significance ( P<0.01) ,and there’ s a tendency that DC would be further reduced by the increasing of PP ( P<0.01).Correlation anylysis from Spearman showed that dynamic PP was negatively correlated with DC (r= -0.68 ,P<0.05) .Conclusions The autonomic nervous function of patients with essential hypertension were damaged ,and the damagement would be worse by the increasing of PP .
