目的探讨机械通气患者脱机困难影响因素，提高脱机成功率。方法对45例脱机困难患者临床资料分析。结果发现患者脱机困难影响因素依次为院患者生理学指标异常、原发病进展及出现严重并发症、医生对脱机时机的把握，呼吸道管理水平，同时患者的精神状态、心理依赖也是影响患者脱机的因素。结论对于脱机困难患者除了积极治疗原发病，控制其并发症，及时纠正患者生理学指标外，临床医生掌握适宜脱机时间窗，严格气道管理、防止呼吸机相关性肺炎发生对患者早日脱机也是非常重要的。

목적탐토궤계통기환자탈궤곤난영향인소，제고탈궤성공솔。방법대45례탈궤곤난환자림상자료분석。결과발현환자탈궤곤난영향인소의차위원환자생이학지표이상、원발병진전급출현엄중병발증、의생대탈궤시궤적파악，호흡도관리수평，동시환자적정신상태、심리의뢰야시영향환자탈궤적인소。결론대우탈궤곤난환자제료적겁치료원발병，공제기병발증，급시규정환자생이학지표외，림상의생장악괄의탈궤시간창，엄격기도관리、방지호흡궤상관성폐염발생대환자조일탈궤야시비상중요적。
Objective To study the dif icult weaning factors of patients with mechanical ventilation, improve weaning success. Methods Clinical analysis of 45 cases of patients with dif icult weaning. Results Found in patients with dif icult weaning factors in the fol owing order:Physiological indexes in patients with abnormal development, primary disease and serious complications, doctors grasp of weaning time, airway management, and patient's state of mind, psychological dependence are also factors that impact patients ' weaning. Conclusion For weaning e problems in addition to treatment of patients with primary disease, control of its complications, cor ect physiological indicators in patients with clinicians acquire suitable for weaning time window, airway management, strict prevention of ventilator-associated pneumonia occur weaning as soon as possible for patients is also very important.
