目的：探讨嗜酸性粒细胞和异常淋巴细胞在恙虫病中的表达情况及诊断意义。方法回顾性分析20例恙虫病患者，并对其血常规、血涂片等进行检测；另外选取健康体格检查者20名作为健康对照组，对照研究恙虫病患者异常淋巴细胞、嗜酸性粒细胞的表达；计量资料比较采用t检验。结果20例恙虫病患者中嗜酸性粒细胞减少18例（90％，18／20），其中嗜酸性粒细胞计数为0占80％（16／20）。异常淋巴细胞检出率95％（19／20），其在各种实验室检查中的检出率居首位。治疗前恙虫病组嗜酸粒细胞计数为（0．0035±0．008）×109／L，显著低于健康对照组（0．133±0．114）×109／L；治疗前异常淋巴细胞百分比为（5±4．22）％，显著高于健康对照组（0．05±0．224）％，差异均有统计学意义（P＜0．05）。经阿奇霉素治疗4～9天后，复查血常规及血涂片，嗜酸性粒细胞计数为（0．079±0．05）×109／L，异常淋巴细胞百分比为（0．25±0．78）％，与治疗前相比差异均有统计学意义（ P＜0．05）。异常淋巴细胞检出率下降至10％（2／20），与治疗前相比差异有统计学意义（P＜0．05）。结论嗜酸性粒细胞减少和异常淋巴细胞的检出率升高是恙虫病患者的重要临床特征之一，对恙虫病早期诊断具有一定的重要意义。
목적：탐토기산성립세포화이상림파세포재양충병중적표체정황급진단의의。방법회고성분석20례양충병환자，병대기혈상규、혈도편등진행검측；령외선취건강체격검사자20명작위건강대조조，대조연구양충병환자이상림파세포、기산성립세포적표체；계량자료비교채용t검험。결과20례양충병환자중기산성립세포감소18례（90％，18／20），기중기산성립세포계수위0점80％（16／20）。이상림파세포검출솔95％（19／20），기재각충실험실검사중적검출솔거수위。치료전양충병조기산립세포계수위（0．0035±0．008）×109／L，현저저우건강대조조（0．133±0．114）×109／L；치료전이상림파세포백분비위（5±4．22）％，현저고우건강대조조（0．05±0．224）％，차이균유통계학의의（P＜0．05）。경아기매소치료4～9천후，복사혈상규급혈도편，기산성립세포계수위（0．079±0．05）×109／L，이상림파세포백분비위（0．25±0．78）％，여치료전상비차이균유통계학의의（ P＜0．05）。이상림파세포검출솔하강지10％（2／20），여치료전상비차이유통계학의의（P＜0．05）。결론기산성립세포감소화이상림파세포적검출솔승고시양충병환자적중요림상특정지일，대양충병조기진단구유일정적중요의의。
Objective To investigate the expression and diagnostic significance of acidophilic granulocyte and abnormal lymphocytes in the tsutsugamushi disease.Methods 20 cases of tsutsugamushi disease were retrospectively analyzed and examined their routine blood and blood smear;another 20 were selected of physical health inspectors as the control group, and next compare the two groups that eosinophils and abnormal lymphocytes express in tsutsugamushi disease patients; Measurement data were compared using t test. Results 20 cases of tsutsugamushi disease patients in eosinophils reduced 18 cases ( 90%, 18/20 ) , which there was 80%of eosinophil count was 0 ( 16/20 ) .The detection rate of abnormal lymphocytes was 95% ( 19/20), and it was in the first place of various laboratory detection rate.Before treatment acidophilic granulocyte count of the tsutsugamushi disease group was (0.0035 ±0.008) ×109/L, which was significantly lower than the healthy control group (0.133 ±0.114) ×109/L; before treating abnormal lymphocyte percentage was (5 ± 4.22)%, which was significantly higher than the control group ( 0.05 ±0.224 )%, the differences were statistically significant (P<0.05).Reviewing the routine blood and blood smear after 4 ~9 days of treatment with azithromycin, eosinophil counting ( 0.079 ±0.05 ) ×109/L and abnormal lymphocyte percentage being (0.25 ±0.78 )%, it turned out that compared with pre-treatment the differences of them were statistically significant (P<0.05).And the detection rate of abnormal lymphocytes decreased to 10% (2/20), compared with before treatment difference was statistically significant ( P <0.05 ).Conclusions The reduction of eosinophils and increasing of detection rate of the abnormal lymphocytes is one of the important clinical features of the tsutsugamushi disease patients, which has some significance for the early diagnosis of the tsutsugamushi disease.
