目的：观察术前静注帕瑞昔布钠对胆囊切除患者术后躁动、疼痛所造成影响。方法：选择本院2013年5月-2014年11月期间所收治56例胆囊结石患者作为研究对象，随机分为两组，观察组29例患者，对照组27例患者。观察组术前静注帕瑞昔布钠，对照组静注氟比洛芬酯，其他麻醉药物的使用相同，比较两组患者术后1、2、4、12、24 h的疼痛情况、躁动发生率、不良反应发生率。结果：观察组患者术后1、2、4、12、24 h的VAS评分均显著低于对照组，提示疼痛感更轻，两组比较差异有统计学意义（P<0.05）。观察组术后躁动发生率为6.8%（2/29），显著低于对照组的29.6%（8/27），两组比较差异有统计学意义（P<0.05）；此外，观察组恶心呕吐、皮肤瘙痒不良反应发生率也明显比对照组低，两组比较均有统计学意义（P<0.05）。结论：帕瑞昔布钠在腹腔镜胆囊切除术中的应用具有较好镇痛效果，患者苏醒期镇静效果显著，同时也有效防止躁动、恶心呕吐等不良反应的出现，应用前景广阔，值得推广。

목적：관찰술전정주파서석포납대담낭절제환자술후조동、동통소조성영향。방법：선택본원2013년5월-2014년11월기간소수치56례담낭결석환자작위연구대상，수궤분위량조，관찰조29례환자，대조조27례환자。관찰조술전정주파서석포납，대조조정주불비락분지，기타마취약물적사용상동，비교량조환자술후1、2、4、12、24 h적동통정황、조동발생솔、불량반응발생솔。결과：관찰조환자술후1、2、4、12、24 h적VAS평분균현저저우대조조，제시동통감경경，량조비교차이유통계학의의（P<0.05）。관찰조술후조동발생솔위6.8%（2/29），현저저우대조조적29.6%（8/27），량조비교차이유통계학의의（P<0.05）；차외，관찰조악심구토、피부소양불량반응발생솔야명현비대조조저，량조비교균유통계학의의（P<0.05）。결론：파서석포납재복강경담낭절제술중적응용구유교호진통효과，환자소성기진정효과현저，동시야유효방지조동、악심구토등불량반응적출현，응용전경엄활，치득추엄。
Objective:To observe the restlessness and pain effects caused by preoperative parecoxib sodium injection on resection of gallbladder patientsMethod:56 cases of cholecystolithiasis patients were treated in our hospital from May 2013 to November 2014 as research object,and were randomly divided into two groups,29 cases in the observation group and 27 cases in the control group.The observation group with preoperative parecoxib sodium, control group with intravenous injection of flurbiprofen axetil, the same of other drugs were used by the two groups.Two groups of patients’ VAS scores after 1, 2, 4, 12, 24 h, the incidence of agitation,and the incidence of adverse reaction were compared.Result:The observation group patients’ VAS scores after 1, 2, 4, 12, 24 h were significantly lower than those in control group, suggested that the pain was lighter,the difference was statistically significant(P<0.05).The incidence of agitation rate of patients in observation group was 6.8% (2/29),significantly lower than the control group of 29.6% (8/27), the difference was statistically significant(P<0.05).In addition, observation group of adverse reaction of nausea and vomiting,and itch of skin were also significantly lower than control group, the difference was statistically significant (P<0.05).Conclusion:Parecoxib sodium has good effect of analgesia in laparoscopic cholecystectomy surgery, sedative effect of analepsia period is significant, but also effectively in prevent restlessness, nausea and vomiting adverse reaction, wide application prospect, is worthy of promotion.
