目的探讨微创25 G高速玻璃体切除眼内异物摘出术不同手术方式的效果.方法回顾性分析2012年1月至2013年6月内施行的25 G微创玻璃体切除磁性眼内异物摘出术25例(25 只眼),术后随访6～24个月,一期硅油填充者,待硅油取出术后观察6个月以上.观察术后视力、眼压及术中术后医源性并发症发生情况.结果所有患者眼内异物均一次顺利摘出,术中术后无医源性并发症发生.术后视力均保持术前视力或有不同程度的提高.结论微创25 G高速玻璃体切除磁性眼内异物摘出术,根据病情采用相应的手术方法,恰当地选择异物摘出口及选择辅助器械,可顺利完成手术,减少医源性并发症发生,收到微创、高效、安全的手术效果.

Objective To investigate the efficacy of different surgical approaches of magnetic intraocular foreign body extraction with 25G high-speed vitrectomy (25G HS-PPV).Methods Clinical data of 25 eyes of 25 patients were retrospectively analyzed.They had undergone extraction of magnetic intraocular foreign body with 25G HS-PPV from January 2012 to June 2013.Each patient was followed up for a period of 6 to 24 months,and the follow up period was 6 months or more after silicone oil removal for patients with silicone oil tamponade.Preoperative visual acuity,postoperative uncorrected visual acuity,postoperative best corrected visual acuity,intraocular pressure (IOP) and complication were observed.Results The magnetic intraocular foreign bodies were successfully extracted in all patients.Postoperative visual acuity maintained the same or improved compared with preoperative visual acuity.No iatrogenic complication occurred.Conclusion Choosing proper surgical approach and technique in 25G high-speed pars plana vitrectomy,magnetic intraocular foreign body could be successfully extracted with less iatrogenic complications.It is a microinvasive,safe and efficient method for magnetic intraocular foreign body extraction.
