目的:探讨酸蚀喷砂处理对微种植体周围骨整合过程中神经生长因子( NGF)表达的影响。方法36枚微种植体随机分为2组,种植体表面进行SLA(酸蚀喷砂)处理为SLA组(n=18),种植体表面未进行处理为对照组(n=18)。选取6只Beagle犬下颌骨为植入区,每只犬一侧植入3枚SLA种植体(实验组),对侧植入3枚未经SLA处理的微种植体(对照组)。在实验初始、4周、6周时分别于下颌骨左右侧各植入1枚,实验8周时处死动物,获得愈合时间8周组、4周组、2周组的微种植体骨标本,并进行免疫组织化学观察及NGF平均光密度值测定。结果免疫组织化学显示2周时实验组和对照组NGF的染色强度最强,4周、8周时实验组染色强度减弱。而对照组4周时染色强度仍较高,8周减弱。2周时实验组NGF平均光密度值高于对照组(P<0.05),4周时实验组低于对照组(P<0.05),8周时NGF平均光密度值实验组与对照组差异无统计学意义(P>0.05)。两两比较发现除对照组2周与4周组无显著差异外(P>0.05),各组间差异均具有统计学意义(P<0.05)。结论对微种植体表面进行SLA处理,可以促进骨整合的形成,具有良好的初期稳定性,可以进行早期负载。

목적:탐토산식분사처리대미충식체주위골정합과정중신경생장인자( NGF)표체적영향。방법36매미충식체수궤분위2조,충식체표면진행SLA(산식분사)처리위SLA조(n=18),충식체표면미진행처리위대조조(n=18)。선취6지Beagle견하합골위식입구,매지견일측식입3매SLA충식체(실험조),대측식입3매미경SLA처리적미충식체(대조조)。재실험초시、4주、6주시분별우하합골좌우측각식입1매,실험8주시처사동물,획득유합시간8주조、4주조、2주조적미충식체골표본,병진행면역조직화학관찰급NGF평균광밀도치측정。결과면역조직화학현시2주시실험조화대조조NGF적염색강도최강,4주、8주시실험조염색강도감약。이대조조4주시염색강도잉교고,8주감약。2주시실험조NGF평균광밀도치고우대조조(P<0.05),4주시실험조저우대조조(P<0.05),8주시NGF평균광밀도치실험조여대조조차이무통계학의의(P>0.05)。량량비교발현제대조조2주여4주조무현저차이외(P>0.05),각조간차이균구유통계학의의(P<0.05)。결론대미충식체표면진행SLA처리,가이촉진골정합적형성,구유량호적초기은정성,가이진행조기부재。
Objective To explore the effect of sand blasted-large grit-acid etched( SLA) surface on the expression of nerve growth fac-tor(NGF)in bone tissues around the micro-implant. Methods Thirty-six micro-implants were divided randomly into two groups. The micro-implant surface was treated by acid etching and sand blasting in experiment group(n=18),while the micro-implant surface was not given any treatment in control group(n=18). Six Beagle dogs were included in this study. In each dog,three micro-implants with SLA were placed in one side of mandible as experiment groups, and three non-SLA micro-implants were implanted in the other side as control groups. One micro-implant was inserted in the mandible implantation zone of each side at the beginning,at 4 week,6 week, re-spectively,and the dogs were sacrificed at 8 week after operation. The implant specimens with different healing time(8 weeks,4 weeks, 2 weeks)were obtained correspondingly,and examined by immunohistochemical(IHC)staining. The average optical density value of NGF was determined. Results Immunohistochemistry showed NGF expression was the highest at 2 week in both experiment group and control group,and decreased at 4 week and 8 week in experimental group, however, the NGF expression was still higher at 4 week but lower at 8 week in control group. The average optical density value was significantly higher in experiment group than that in control group at 2 week(P<0. 05), but it was significantly lower in experiment group than that in control group at 4 week(P<0. 05). No sta-tistical difference was found in NGF expression between experiment group and control group at 8 week. There was statistical difference at different time points between experiment group and control group(P<0. 05), except between 2 week and 4 week in control group(P>0. 05). Conclusion SLA micro-implant surface could promote the osseointegration formation, which has a good primary stability and is reliable for early loading.
