目的:探讨沙培林(OK-432)和岩舒(复方苦参注射液)胸腔内给药对恶性胸腔积液中血管内皮生长因子(VEGF)及外周血TNF-α、IL-2的影响及其机制。方法88例恶性胸腔积液患者分为两组,对照组胸腔内注入沙培林,试验组胸腔内序贯注入沙培林和岩舒,对比疗效,检测胸水VEGF及外周血TNF-α、IL-2浓度。结果试验组治疗有效率为81.8%,高于对照组的61.3%(P<0.05);两组患者治疗后胸水VEGF浓度较治疗前降低(P<0.05),外周血TNF-α、IL-2浓度较治疗前升高(P<0.05);两组治疗后对比,试验组胸水VEGF浓度较对照组降低(P<0.05),外周血TNF-α、IL-2浓度较对照组升高(P<0.05)。结论沙培林联合岩舒胸腔内用药能有效治疗恶性胸腔积液,降低胸水VEGF浓度及升高外周血TNF-α、IL-2浓度。

목적:탐토사배림(OK-432)화암서(복방고삼주사액)흉강내급약대악성흉강적액중혈관내피생장인자(VEGF)급외주혈TNF-α、IL-2적영향급기궤제。방법88례악성흉강적액환자분위량조,대조조흉강내주입사배림,시험조흉강내서관주입사배림화암서,대비료효,검측흉수VEGF급외주혈TNF-α、IL-2농도。결과시험조치료유효솔위81.8%,고우대조조적61.3%(P<0.05);량조환자치료후흉수VEGF농도교치료전강저(P<0.05),외주혈TNF-α、IL-2농도교치료전승고(P<0.05);량조치료후대비,시험조흉수VEGF농도교대조조강저(P<0.05),외주혈TNF-α、IL-2농도교대조조승고(P<0.05)。결론사배림연합암서흉강내용약능유효치료악성흉강적액,강저흉수VEGF농도급승고외주혈TNF-α、IL-2농도。
Objective To explore the influence of VEGF, TNF-αand IL-2 levels of intrathoracic perfusion of OK-432 combined with compound Kushen injection in teatment of malignant pleural effusion. Methods 88 patients with malignant pleural effusion were randomly divided into 2 groups: control group treated with OK-432, test group treated with OK-432 com-bined with compound Kushen injection. The responses of pleural effusion were observed. The concentrations of VEGF in pleural effusion, TNF-αand IL-2 in peripheral blood were detected by ELISA. Results The effective rate in test group(81.8%) was higher than that in control group(61.3%), P<0.05. The concentrations of VEGF in pleural effusion reduced, TNF-αand IL-2 in peripheral blood significantly increased after the treatment in both groups (P<0.05). The concentration of VEGF in pleural effu-sion was lower in test group than control group (P<0.05), TNF-αand IL-2 in peripheral blood were significantly higher in test group than control group(P<0.05). Conclusion Treatment of malignant pleural effusion with intrathoracic OK-432 and com-pound Kushen injection has good efficacy. It can enhance the expression of TNF-αand interleukin-2, and lower VEGF level in pleural effusion.
