目的:探讨简化客观结构化临床考试模式应用于ICU专科护士培训综合能力测评的可行性。方法在ICU专科护士培训考核体系中应用简化客观结构化临床考试考核模式,入学、结业均给予理论考试,结业前给予简化客观结构化临床考试多站式考核,结合培训内容设置情景,编写考核病例,选拔及培训标准化病人和设置客观结构化临床考试考核站点考核2012—2014年3届共100名培训学员并进行成绩分析和问卷调查。结果受各学员学历、资历的影响,入学时理论考试及格率仅为44%。结业考试时,客观结构化临床考试各站点考核合格率为100%,90%的学员认为此次客观结构化临床考试病例结构安排合理,93%的学员认为考站及时间安排合理;86%的学员表示该考核适应临床实际;88%的学员认为能反映临床护理综合能力;89%的学员认为该考核模式公平。结论应用简化客观结构化临床考试能检验学员理论知识、操作技能、沟通能力及应变能力等,能客观检验学员是否具有能够胜任紧张而繁琐的ICU工作的综合能力。但该考核成绩受培训场地、培训时间及主考老师和标准化病人的主观影响较大,尚有待改进。

Objective To explore the feasibility of simplified OSCE (objective structured clinical examination) in comprehensive competence assessment of ICU nurse specialists. Methods We introduced simplified OSCE into training and assessment system for ICU nurse specialists. There were two theory examinations and simplified OSCE after training. Based on training contents for ICU nurse specialists, we set up scenarios, composed the medical records for the scenarios, chose and trained standardized patients (SPs), and developed several OSCE stations which were sequentially entered by 100 ICU nurse specialists from 2012 to 2014 for competence assessment after receiving training. OSCE scores were analyzed and nurses were surveyed about the effect of OSCE. ResuIts For different educational background and qualification of nurses, the pass rate of the first theory exam was only 44% while that of every testing reached 100% after training. Of the total, 90% of the nurses thought case structure arrangement of OSCE reasonable and 93% of them test station and time arrangement reasonable. For 86% of the participants, OSCE was practical for clinical practice and 88%of the nurses regarded it could reflect the comprehensive ability in clinical practice meanwhile 89%of them thought it was fair for participants. ConcIusion The simplified OSCE can test students’ integrated ability and is effective for choosing a qualified nurse specialist in ICU. But the test result is influenced by training site, training time, the examiner and the SPs so it remains to be improved.
