目的:介绍前臂双静脉皮瓣在手指近中节指腹皮肤并指动脉缺损的临床应用。方法2010年1月—2013年12月,应用包含两条静脉的前臂静脉皮瓣,修复2—5指近中节,拇指近节皮肤并指动脉缺损68例85指,皮瓣切取面积为1.5 cm×2.0 cm~3.0 cm×6.0 cm。结果术后皮瓣全部成活,随访6~12个月,手指外形满意,运动功能良好。修复皮瓣感觉恢复不理想,两点辨别觉6~12 mm,平均8 mm ,供区无明显并发症。结论游离的含双静脉的前臂静脉皮瓣可用于手指皮肤并指动脉缺损的修复重建,该皮瓣具有取材方便、厚薄适中、柔韧性好的优点,感觉恢复不良是其主要缺陷。

목적:개소전비쌍정맥피판재수지근중절지복피부병지동맥결손적림상응용。방법2010년1월—2013년12월,응용포함량조정맥적전비정맥피판,수복2—5지근중절,무지근절피부병지동맥결손68례85지,피판절취면적위1.5 cm×2.0 cm~3.0 cm×6.0 cm。결과술후피판전부성활,수방6~12개월,수지외형만의,운동공능량호。수복피판감각회복불이상,량점변별각6~12 mm,평균8 mm ,공구무명현병발증。결론유리적함쌍정맥적전비정맥피판가용우수지피부병지동맥결손적수복중건,해피판구유취재방편、후박괄중、유인성호적우점,감각회복불량시기주요결함。
Objective To introduce the clinical application of forearm double venous flap in the treatment of the proximal and middle phalanx palmar skin defects associated finger artery damage. Methods From January 2010 to December 2012, 85 fingers in 68 cases including proximal and middle phalanx finger or proximal thumb palmar skin defects associated finger artery damage were repaired by forearm venous flap containing two veins. The area of flaps were from 1.5 cm×2.0 cm to 3.0 cm×6.0 cm. Results All the flaps survived after transplantation. Both the shape and function of wounded fingers were satisfied with follow-up of 6 to 12 months. Sensory recovery was not ideal and the distance of two-point discrimination ranged from 6 to 12 mm, with average 8 mm. There were no obvious complications in donor area. Conclusion Free forearm venous flap with double veins can be applied to reconstruct palmar skin defects of fingers associated artery damage. The adavantages of the flap is convenient for harvest, moderate of thickness, and good flexibility. However the poor sensory recovery is the main drawback.
