目的：研究慢性病自我管理项目(CDSMP)在胃癌生物免疫治疗患者中的应用效果。方法选取2011年3月—2014年3月进行生物免疫治疗的126例胃癌患者作为研究对象,按照随机数字表法随机分为研究组和对照组,每组各63例。研究组患者实施 CDSMP,对照组由护士进行常规护理。比较两组患者干预后自我效能、疾病症状自我管理、健康状况自评和并发症发生率的差异。结果干预后研究组患者自我效能得分为(18.52±3.21)分,高于对照组的(13.46±5.43)分,差异有统计学意义(t =2.351,P <0.05)。研究组患者认知症状管理、运动与锻炼、医患沟通、治疗依从度得分分别为(4.57±1.23)分,(428.57±28.15)min,(4.85±0.58)分,(4.64±1.56)分,均高于对照组,差异有统计学意义(t 值分别为2.056,2.379,2.114,2.009；P <0.05)。研究组患者整体健康自评、精力指标、健康担忧及疲乏感得分分别为(4.73±2.18),(4.57±1.36),(4.85±0.58),(4.64±1.56)分,均高于对照组,差异有统计学意义(t 值分别为2.533,2.552,2.602,2.499；P <0.05)。两组患者梗阻、出血及穿孔的发生率比较,差异无统计学意义(P >0.05)。结论慢性病自我管理项目对胃癌生物免疫治疗患者病情的缓解与康复具有明显的促进作用。

목적：연구만성병자아관리항목(CDSMP)재위암생물면역치료환자중적응용효과。방법선취2011년3월—2014년3월진행생물면역치료적126례위암환자작위연구대상,안조수궤수자표법수궤분위연구조화대조조,매조각63례。연구조환자실시 CDSMP,대조조유호사진행상규호리。비교량조환자간예후자아효능、질병증상자아관리、건강상황자평화병발증발생솔적차이。결과간예후연구조환자자아효능득분위(18.52±3.21)분,고우대조조적(13.46±5.43)분,차이유통계학의의(t =2.351,P <0.05)。연구조환자인지증상관리、운동여단련、의환구통、치료의종도득분분별위(4.57±1.23)분,(428.57±28.15)min,(4.85±0.58)분,(4.64±1.56)분,균고우대조조,차이유통계학의의(t 치분별위2.056,2.379,2.114,2.009；P <0.05)。연구조환자정체건강자평、정력지표、건강담우급피핍감득분분별위(4.73±2.18),(4.57±1.36),(4.85±0.58),(4.64±1.56)분,균고우대조조,차이유통계학의의(t 치분별위2.533,2.552,2.602,2.499；P <0.05)。량조환자경조、출혈급천공적발생솔비교,차이무통계학의의(P >0.05)。결론만성병자아관리항목대위암생물면역치료환자병정적완해여강복구유명현적촉진작용。
Objective To investigate the application effects of self-management of chronic disease program (CDSMP) in gastric cancer patients with biological immune therapy. Methods Totals of 126 gastric cancer patients with biological immune therapy were selected and randomly divided into experimental group and control group, with 63 cases in each. Patients in the experimental group were carried out the CDSMP, while patients in the control group were implemented the routine care by nurses. Subsequently, the self-efficacy, symptoms of the disease management, health status and the incidence of complications were analyzed and compared in the two groups. Results After the treatment, the score of self-efficacy of the experimental group was (18. 52 ± 3. 21), which was significantly higher than that of (13. 46 ± 5. 43) in the control group (t =2. 351, P < 0. 05). The scores of management of disease symptoms, doctor patient communication, treatment adherence of the experimental group were (4. 57 ± 1. 23), (4. 85 ± 0. 58) and (4. 64 ± 1. 56), which were significantly higher than those of the control group (t =2.056,2.114,2.009, respectively;P <0. 05). The scores of overall health self-assessment, vigor indicates, health anxiety and fatigue of the experimental group were (4. 73 ± 2. 18), (4.57 ±1.36), (4.85 ±0.58) and (4.64 ±1.56), which were significantly higher than those of the control group (t =2.533,2. 552,2. 602,2. 499, respectively;P < 0. 05). Additionally, the incidence of complications was similar and the difference was not statistically significant (P >0.05). Conclusions DSMP could promote the relief and rehabilitation of gastric cancer patients with biological immune therapy.
