目的：研究特利加压素联合生长抑素治疗老年下消化道大出血的临床效果。方法选取本院2010年9月~2013年9月收治的96例下消化道大出血老年患者，按随机抽样法将所有患者分为两组，各48例。对照组单纯应用生长抑素进行治疗，观察组在应用生长抑素的基础上加用特利加压素治疗。对比两组患者的治疗情况、不良反应及预后等。结果观察组总有效率（95.83%）明显高于对照组（83.33%），死亡率（2.08%）低于对照组（10.42%），差异有统计学意义（P<0.05）；治疗过程中，观察组不良反应发生率为10.42%，与对照组的8.33%比较差异无统计学意义（P>0.05）；1年后随访，观察组再出血发生率和死亡率分别为4.26%、4.26%，远低于对照组的13.95%、13.95%，差异有统计学意义（P<0.05）。结论特利加压素联合生长抑素治疗老年下消化道大出血的效果显著，患者预后好，安全性高，值得临床推广应用。

Objective To study the clinical effect of terlipressin combined with somatostatin in treatment of the aged with lower gastrointestinal bleeding. Methods 96 elderly patients with lower gastrointestinal bleeding in our hospital from September 2010 to September 2013 were selected and randomly divided into two groups,48 cases in each group. Somatostatin was used in control group,somatostatin combined with terlipressin were applied in observation group.The treatment outcome,adverse reaction and prognosis between two groups were compared. Results Total effective rate of observation group (95.83%)was significantly higher than that of control group (83.33%),death rate of observation group (2.08%)was lower than that of control group (10.42%),with significant difference (P<0.05).The incidence rate of adverse reaction in observation group (10.42%) had no significant difference compared with that in control group (8.33%) (P>0.05).Upon the follow-up after 1 year,the incidence rate of rehaemorrhagia and death rate in observation group (4.26%, 4.26%)was significantly lower than that in control group(13.95%,13.95%) respectively(P<0.05). Conclusion Terlipressin combined with somatostatin in treatment of elderly patients with lower digestive tract hemorrhage has significant effect, the prognosis is good,high safety.It is worthy of clinical promotion and application.
