目的:探讨脊髓损伤后应用甲泼尼松龙对大鼠行为学及组织病理学的作用。方法将60只雄性SD大鼠随机分成A、B、C三组,A组为正常对照组,B组为损伤对照组,C组为实验组,每组20只。 A组采用单纯全椎板切除,不损伤脊髓;B、C组采用Allen法制作T10脊髓损伤模型。 C组首计量以30 mg/kg甲泼尼松龙尾静脉注射,余计量以5.4 mg/(kg·h)静脉给药,共23 h;A、B组以生理盐水代替甲泼尼松龙尾静脉注射,剂量同C组。术后1 d、1周、2周、4周对三组大鼠后肢运动功能行为进行BBB评分及斜板试验;再在术后1 d和4周各取10只大鼠受损节段脊髓行HE染色及光镜下观察。结果 C组大鼠在损伤后各时间点BBB评分和斜板试验角度显著高于B组,差异有统计学意义(P<0.01)。组织病理学显示脊髓损伤后大鼠脊髓组织出血、水肿,空洞形成,神经组织溶解消失,明显脱髓鞘改变。C组病理改变较B组轻。结论甲泼尼松龙可以抑制脊髓损伤后炎症反应,减少组织坏死及空洞范围,改善大鼠运动行为功能。

목적:탐토척수손상후응용갑발니송룡대대서행위학급조직병이학적작용。방법장60지웅성SD대서수궤분성A、B、C삼조,A조위정상대조조,B조위손상대조조,C조위실험조,매조20지。 A조채용단순전추판절제,불손상척수;B、C조채용Allen법제작T10척수손상모형。 C조수계량이30 mg/kg갑발니송룡미정맥주사,여계량이5.4 mg/(kg·h)정맥급약,공23 h;A、B조이생리염수대체갑발니송룡미정맥주사,제량동C조。술후1 d、1주、2주、4주대삼조대서후지운동공능행위진행BBB평분급사판시험;재재술후1 d화4주각취10지대서수손절단척수행HE염색급광경하관찰。결과 C조대서재손상후각시간점BBB평분화사판시험각도현저고우B조,차이유통계학의의(P<0.01)。조직병이학현시척수손상후대서척수조직출혈、수종,공동형성,신경조직용해소실,명현탈수초개변。C조병리개변교B조경。결론갑발니송룡가이억제척수손상후염증반응,감소조직배사급공동범위,개선대서운동행위공능。
Objective To study effect on behavioristics and histopathology of methylprednisolone in the treatment of rat with spinal cord injury. Methods Sixty SD male rats were randomly divided into three groups,each group had 20 rats. Group A was normal control group,group B was injury control group,and group C was treatment group.Rats’ spine in group A was merely respected by total laminectomy without any hurts to its spinal cord.Both group B and C were as-signed to be evaluated by using spinal cord injury model (T10) made by Allen method.Rats in group C were injected methylprednisolone which amounted to 30 mg/kg at the beginning.However,the remalning were injected at the amount of 5.4 mg/kg each time,23 hours to the end.Methylprednisolone was replaced by normal saline instead in group A and B,the same dose of group C.After operation of 1 day,1 week,2 weeks and 4 weeks,the hind limb motor function includ-ing BBB score and inclined plane test among three groups were applied.After operation of 1 day and 4 weeks,injured spinal cord of 10 rats were observed under microscopy after HE-stalning. Results BBB score and inclined plane test angle in group C was significantly higher than that in group B at each time point respectively,with statistical difference (P<0.01).Histopathological study showed that hemorrhage and edema in the spinal cord after injury,and vacuolation in cells,nervous tissue melted and demyelination changed.Pathological changed of group C was lighter than that in group B. Conclusion Methylprednisolone can restraln inflammatory response of spinal cord injury,reduce range of cavity and tissue necrotic,improve function of behavior.
