目的对临床怀疑胰腺癌而首次超声内镜下细针穿刺( EUS-FNA)诊断不明确的患者，重复行EUS-FNA细胞学检查和18F-氟脱氧葡萄糖(18F-FDG)正电子发射断层显像(PET)-CT扫描检查对胰腺癌诊断的对比研究。方法收集首次EUS-FNA结果不明确重复EUS-FNA和PET-CT的患者，进行配比分析。结果重复EUS-FNA的准确率为80%，敏感性为72.7%，特异性为100%，阳性预测值为100%，阴性预测值为57.1%；PET-CT检查的准确率为75%，敏感性为66.7%，特异性为83.3%，阳性预测值为80%，阴性预测值为71.4%。结论临床怀疑胰腺癌而首次EUS-FNA结果不明确时，重复EUS-FNA和PET-CT都是可行的方法，重复EUS-FNA略优于PET-CT检查。
목적대림상부의이선암이수차초성내경하세침천자( EUS-FNA)진단불명학적환자，중복행EUS-FNA세포학검사화18F-불탈양포도당(18F-FDG)정전자발사단층현상(PET)-CT소묘검사대이선암진단적대비연구。방법수집수차EUS-FNA결과불명학중복EUS-FNA화PET-CT적환자，진행배비분석。결과중복EUS-FNA적준학솔위80%，민감성위72.7%，특이성위100%，양성예측치위100%，음성예측치위57.1%；PET-CT검사적준학솔위75%，민감성위66.7%，특이성위83.3%，양성예측치위80%，음성예측치위71.4%。결론림상부의이선암이수차EUS-FNA결과불명학시，중복EUS-FNA화PET-CT도시가행적방법，중복EUS-FNA략우우PET-CT검사。
Objective To comparatively evaluate the diagnostic value of repeated Endoscopic Ultrasound-guided Fine-needle Aspiration (EUS-FNA) and 18F-FDG PET-CT among patients with suspicion for pancreatic cancer. Method Patients with suspected pancreatic cancer after unclear results of the first EUS-FNA cytology were recruited from our hospital, Second Military Medical University during Jan. 2008 and Oct. 2014. The information of repeated EUS-FNA and PET-CT imaging were collected respectively ratio analysis. Result The accuracy, sensitivity, specificity, positive predictive value and negative predictive value were 80%, 72. 7%, 100%, 100% and 57. 1% for repeated EUS-FNA, and 75%, 66. 7%, 83. 3%, 80% and 71. 4% for PET-CT respectively. Conclusion For the patients of suspected pancreatic cancer who first performed EUS-FNA cytology with unclear results, repeated EUS-FNA is slightly better than PET-CT as a further examination for diagnosis.
