目的 观察内质网应激在脓毒症大鼠肾损伤中的作用.方法 32只SD雄性大鼠随机平均分为假手术组、脓毒症6h组、12 h组和24h组,采用盲肠结扎穿刺法建立大鼠脓毒症肾损伤模型,分别于术后6、6、12、24h处死大鼠.观察大鼠肾功能,原位末端转移酶标记(TUNEL)法检测肾脏细胞凋亡指数(AJ),Western blot和反转录-聚合酶链反应(RT-PCR)检测肾组织葡萄糖调节蛋白78(GPR78)、CAAT/增强子结合蛋白同源蛋白(CHOP)和半胱氨酸天冬氨酸蛋白酶-12(Caspase-12)蛋白和mRNA表达.结果 与假手术组比较,脓毒症6h组尿素氮[(7.54±1.62) mmol/L比(5.26±0.34) mmol/L]、血肌酐[(57.14±9.53) μmol/L比(42.35±4.71) μmol/L]、肾脏AI[(25.7±2.8)%比(2.9±0.6)%]以及GPR78、CHOP和Caspase-12蛋白[(0.22±0.03)比(0.14±0.02);(0.16±0.04)比(0.07±0.01);(0.24±0.03)比(0.11±0.02)]及mRNA[(0.31 ±0.08)比(0.21±0.06);(0.33±0.04)比(0.26±0.03);(0.39±0.06)比(0.22±0.02)]表达均升高(P<0.05);而脓毒症12 h组和24h组,尿素氮和血肌酐、肾脏AI、GPR78、CHOP和Caspase-12蛋白和mRNA表达均进一步升高(P<0.05).结论 脓毒症大鼠肾损伤与内质网应激后凋亡途径诱导肾小管上皮细胞凋亡有关.

목적 관찰내질망응격재농독증대서신손상중적작용.방법 32지SD웅성대서수궤평균분위가수술조、농독증6h조、12 h조화24h조,채용맹장결찰천자법건립대서농독증신손상모형,분별우술후6、6、12、24h처사대서.관찰대서신공능,원위말단전이매표기(TUNEL)법검측신장세포조망지수(AJ),Western blot화반전록-취합매련반응(RT-PCR)검측신조직포도당조절단백78(GPR78)、CAAT/증강자결합단백동원단백(CHOP)화반광안산천동안산단백매-12(Caspase-12)단백화mRNA표체.결과 여가수술조비교,농독증6h조뇨소담[(7.54±1.62) mmol/L비(5.26±0.34) mmol/L]、혈기항[(57.14±9.53) μmol/L비(42.35±4.71) μmol/L]、신장AI[(25.7±2.8)%비(2.9±0.6)%]이급GPR78、CHOP화Caspase-12단백[(0.22±0.03)비(0.14±0.02);(0.16±0.04)비(0.07±0.01);(0.24±0.03)비(0.11±0.02)]급mRNA[(0.31 ±0.08)비(0.21±0.06);(0.33±0.04)비(0.26±0.03);(0.39±0.06)비(0.22±0.02)]표체균승고(P<0.05);이농독증12 h조화24h조,뇨소담화혈기항、신장AI、GPR78、CHOP화Caspase-12단백화mRNA표체균진일보승고(P<0.05).결론 농독증대서신손상여내질망응격후조망도경유도신소관상피세포조망유관.
Objective To investigate effect of endoplasmic reticulum stress against kidney injury in rat with sepsis.Methods Thirty-two male SD rats were randomly divided into sham group,sepsis 6 h group,sepsis 12 h group and sepsis 24 h group,with 8 rats in each group.Sepsis was induced by cecal ligation and puncture,Eight rats were sacrificed at 6,6,12 and 24 hours after operation respectively in each group.Levels of blood urea nitrogen (BUN) and Serum creatinine (Scr) were detected.Apoptotic index of kidney issue were measured by terminal deoxynucleotidyl transferase dUTP nick end labeling (TUNEL),and expression levels of glucose regulated protein 78 (GRP78),CCAAT/enhancer-binding protein homologous protein (CHOP) and caspase-12 protein and mRNA was assayed respectively by Western blotting and reverse transcriptase-polymerase chain reaction (RT-PCR).Results Compared to sham group,levels of BUN [(7.54 ± 1.62) mmol/L vs.(5.26 ±0.34) mmol/L],Scr [(57.14 ± 9.53) μmol/L vs.(42.35 ±4.71) μmol/L],AI of kidney issue [(25.7 ±2.8)% vs.(2.9 ± 0.6)%] and expression levels of GRP78,CHOP and caspase-12 protein [(0.22 ±0.03) vs.(0.14 ± 0.02),(0.16 ±0.04) vs.(0.07 ±0.01),(0.24±0.03) vs.(0.11 ±0.02)] and mRNA [(0.31 ± 0.08) vs.(0.21 ±0.06),(0.33±0.04) vs.(0.26±0.03),(0.39±0.06) vs.(0.22±0.02)] were increased (P < 0.05).Levels of BUN,Scr,AI of kidney issue and expression levels of GRP78,CHOP and caspase-12 protein] and mRNA were increased in both sepsis 12 h group and sepsis 24 h group compared to sepsis 6 h group (P <0.05),and these index were higher in sepsis 24 h group than in sepsis 12 h group.Conclusion There exist association between septic kidney injury and endoplasmic reticulum stress-induced apoptosis in kidney issue.
