冠心病的发病率在我国逐年递增，已成为老年患者死亡的主要原因。老年冠心病有其特殊的症状、病因病机及治疗特点，临床症状复杂且多不典型，程度较重且预后偏差，合并症较多，治疗棘手。其病因西医认为是冠状动脉粥样硬化，粥样硬化原因虽不完全明确，但各种危险因素皆对老年人不利。冠心病属于中医“胸痹”范畴，人届老年五脏六腑每露虚象，且“五脏之滞皆为心痛”，老年冠心病的病因中医认为以实少、虚多，标实本虚为主。对于老年冠心病的治疗西医重视老年人的生理特点，因老年人对药物的敏感性改变（减弱或增强），及对药物副作用的反应增加，故用药要严密跟踪效果及不良反应，及时调整。对于针对老年人的PTCA，也要严格评估患者的身体各项指标，不能盲目追求手术治疗；中医则以“补虚辅以祛实”为治疗大法，不独着眼于心，重视心与他脏关系，辨证论治。
