目的:探讨低分子肝素治疗新生儿脓毒症并弥散性血管内凝血( DIC)的临床疗效及应用价值。方法选择宁波大学医学院附属医院治疗的新生儿脓毒症并DIC患儿84例,随机分为观察组和对照组各42例。对照组给予西医常规治疗,观察组在对照组基础上联合低分子肝素治疗,观察两组患儿的临床疗效。结果观察组新生儿DIC指标恢复正常时间、出血停止时间、肝素使用时间、重症监护室( ICU)住院时间与对照组相比,差异均有统计学意义( t值分别为10.1390、12.6781、9.9306和15.4051,均P<0.001)。观察组治疗后C-反应蛋白、降钙素原与对照组治疗后比较,差异均具有统计学意义( t值分别为6.5706和5.0614,均P<0.001)。观察组治疗后部分凝血酶原时间、D-二聚体、血小板计数与对照组相比,差异均具有统计学意义( t值分别为15.1883、15.3153和7.6557,均P<0.001)。结论低分子肝素可以有效改善新生儿脓毒症合并弥散性血管内凝血患儿凝血功能,减轻体内炎症反应,缩短ICU住院时间,值得在临床大力推广使用。

Objective To investigate the clinical efficacy and value of low molecular weight heparin (LMWH) on treatment of neonatal sepsis complicated with disseminated intravascular coagulation ( DIC) .Methods Eighty-four cases of neonatal sepsis complicated with DIC treated in Affiliated Hospital of Medical College of Ningbo University were randomly divided into control group and observation group with 42 cases in each.The control group received conventional treatment, while the observation group was given LMWH therapy additionally. The clinical efficacy of two groups was observed.Results After treatment, there were significant differences in time for neonatal DIC index returning to normal, bleeding stop time, heparin using time and duration in ICU between two groups ( t value was 10.1390, 12.6781, 9.9306 and 15.4051, respectively, all P<0.001).Compared with the control group, CRP and PCT were significantly different in the observation group (t value was 6.5706 and 5.0614, respectively, both P<0.001).Partial prothrombin time, D-dimer concentration and platelet count were remarkably different between two groups (t value was 15.1883, 15.3153 and 7.6557, respectively, all P<0.001). Conclusion LMWH in the treatment of neonatal sepsis with DIC can improve coagulation function effectively, lessen inflammatory reaction and reduce ICU hospitalization length.It is worthy of clinical application.
