目的:探讨闪光视觉诱发电位技术(FVEP)监测双侧大脑半球颅内压差对脑挫裂伤患者预后的作用。方法前瞻性随机将2013—2014年解放军153医院收治的60例脑挫伤患者分为实验组与对照组,对照组根据颅内压值采取相关治疗措施,实验组根据颅内压值和双侧大脑半球颅压差(D值)采取相关治疗,对2组的预后进行对比分析。结果实验组较对照组提前发现病情变化(P=0.003);手术患者行手术前实验组较对照组CT计分低(P=0.034),实验组预后良好率显著高于对照组( P<0.025)。结论应用闪光视觉诱发电位技术监测双侧大脑半球颅内压差有助于提前发现病情变化,及时给予治疗,减少脑组织损伤,有利于脑挫裂患者预后,具有重要的临床价值。

목적:탐토섬광시각유발전위기술(FVEP)감측쌍측대뇌반구로내압차대뇌좌렬상환자예후적작용。방법전첨성수궤장2013—2014년해방군153의원수치적60례뇌좌상환자분위실험조여대조조,대조조근거로내압치채취상관치료조시,실험조근거로내압치화쌍측대뇌반구로압차(D치)채취상관치료,대2조적예후진행대비분석。결과실험조교대조조제전발현병정변화(P=0.003);수술환자행수술전실험조교대조조CT계분저(P=0.034),실험조예후량호솔현저고우대조조( P<0.025)。결론응용섬광시각유발전위기술감측쌍측대뇌반구로내압차유조우제전발현병정변화,급시급여치료,감소뇌조직손상,유리우뇌좌렬환자예후,구유중요적림상개치。
Objective To explore the effect of flash visual evoked potential (FVEP) technology on monitoring intracranial pressure difference of bilateral cerebral hemisphere in patients with cerebral contusion .Methods 60 conscious patients in the 153rd Hospital of PLA from 2013 to 2014 were prospectively randomly divided into experimental group and control group ,the control group only received related therapy based on intracranial pressure ,while the experimental group was given therapeutic treatment according to intracranial pressure and D value .Results Condition changes in experimental group were found earlier than that in control group(P= 0 .003);The CT scores before surgery in experimental group were less than that in control group(P=0 .034);The prognosis in experimental group was better than that in control group(P< 0 .025) .Conclusion In‐tracranial pressure monitored by FVEP can do good to defect changes of illness ,take therapeutic interventions timely and re‐duce injury ,which has beneficial effect on the prognosis of cerebral contusion .
