目的：探讨双联抗血小板治疗非心源性缺血性脑卒中的临床疗效和安全性。方法选取我院就诊的非心源性缺血性脑卒中患者150例，随机分为2组，每组75例。2组均给予相同的基础治疗以及强化他汀类药物治疗，在此基础上，试验组于入院当日口服拜阿司匹林300 mg ，硫酸氢氯吡格雷300 mg ，第2天开始每日口服拜阿司匹林100 mg ，硫酸氢氯吡格雷75 mg ；对照组于入院当天口服拜阿司匹林300 mg ，第2天开始改为每日口服拜阿司匹林100 mg。治疗4周后，观察比较2组的治疗效果、实验室指标、不良反应发生率、复发率情况。结果试验组总有效率85.33％，对照组为65.33％，差异有统计学意义（P＜0.05）；试验组血小板聚集率、血浆黏度、全血低切黏度明显低于对照组；2组不良反应发生率无明显差异（P＞0.05）；试验组2例复发，复发率为2.67％，对照组10例复发，复发率13.33％，试验组复发率低于对照组（ P＜0.05）。结论双联抗血小板治疗非心源性脑卒中临床疗效满意，复发率较低，且无明显不良反应，安全性良好。
목적：탐토쌍련항혈소판치료비심원성결혈성뇌졸중적림상료효화안전성。방법선취아원취진적비심원성결혈성뇌졸중환자150례，수궤분위2조，매조75례。2조균급여상동적기출치료이급강화타정류약물치료，재차기출상，시험조우입원당일구복배아사필림300 mg ，류산경록필격뢰300 mg ，제2천개시매일구복배아사필림100 mg ，류산경록필격뢰75 mg ；대조조우입원당천구복배아사필림300 mg ，제2천개시개위매일구복배아사필림100 mg。치료4주후，관찰비교2조적치료효과、실험실지표、불량반응발생솔、복발솔정황。결과시험조총유효솔85.33％，대조조위65.33％，차이유통계학의의（P＜0.05）；시험조혈소판취집솔、혈장점도、전혈저절점도명현저우대조조；2조불량반응발생솔무명현차이（P＞0.05）；시험조2례복발，복발솔위2.67％，대조조10례복발，복발솔13.33％，시험조복발솔저우대조조（ P＜0.05）。결론쌍련항혈소판치료비심원성뇌졸중림상료효만의，복발솔교저，차무명현불량반응，안전성량호。
Objective To investigate the efficacy and safety of dual antiplatelet therapy for patients with non‐cardiac ische‐mic stroke.Methods 150 cases with non‐cardiac ischemic stroke treated in our hospital were randomly divided into two groups , 75 cases in each group. Besides routine treatment and intensive statin‐related drugs ,the experimental group took aspirin with dosage of 300 mg and clopidogrel bisulfate with 300 mg orally on the day of admission;and received aspirin 100 mg and clopi‐dogrel bisulfate 75 mg daily from the 2nd day. The control group was given therapy based on experimental group without clopi‐dogrel bisulfate. The curative effect ,laboratory indexes ,incidence of adverse events and the recurrence rate of two groups were compared after 4‐week treatment.Results The total effective rate was 85.33% in experimental group and 65.33% in control group ,with statistical difference(P< 0.05). Platelet aggregation rate ,plasma viscosity ,whole blood viscosity at low shear rates of the experimental group were significantly lower than that of control group after treatment. The incidence of adverse e‐vents of two groups showed no significant difference. 2 cases of the experimental group relapsed and the recurrence rate was 2.67% ,while 10 cases of the control group relapsed and the recurrence rate was 13.33% ,and the difference had statistical sig‐nificance(P<0.05).Conclusion Dual anti‐platelet therapy has satisfactory effect on curative efficacy and safety in patients with non‐cardiac ischemic stroke ,with low recurrence rate and no significant adverse reactions.
