目的:通过静息态功能磁共振探讨早期精神分裂症患者脑功能低频振幅变化。方法:对首次发病未用药早期精神分裂症患者(急性期组)、经药物治疗临床症状缓解的早期精神分裂症首次发病患者(缓解期组)以及性别、年龄、受教育年限匹配的健康对照者(对照组)各17例,进行静息态功能磁共振扫描,并计算低频振幅( ALFF),比较3组ALFF变化。结果:三组间ALFF有差异的脑区为左侧直回、梭状回/舌回、右侧颞下回、左侧顶叶、枕叶、楔叶、中央后回、小脑;与健康对照组相比,两病例组ALFF降低的脑区集中在左侧半球,为左侧顶叶小叶,左侧颞下回;而缓解期与急性期组相比,ALFF增加的脑区为右侧壳核,ALFF降低的脑区为直回、楔前叶、梭状回/舌回、左侧颞叶、左侧中央前回和后回、左侧枕中回。结论:精神分裂症患者早期阶段存在多个脑区静息态低频振幅异常,低频振幅降低的脑区主要分布在左侧大脑半球。
目的:通過靜息態功能磁共振探討早期精神分裂癥患者腦功能低頻振幅變化。方法:對首次髮病未用藥早期精神分裂癥患者(急性期組)、經藥物治療臨床癥狀緩解的早期精神分裂癥首次髮病患者(緩解期組)以及性彆、年齡、受教育年限匹配的健康對照者(對照組)各17例,進行靜息態功能磁共振掃描,併計算低頻振幅( ALFF),比較3組ALFF變化。結果:三組間ALFF有差異的腦區為左側直迴、梭狀迴/舌迴、右側顳下迴、左側頂葉、枕葉、楔葉、中央後迴、小腦;與健康對照組相比,兩病例組ALFF降低的腦區集中在左側半毬,為左側頂葉小葉,左側顳下迴;而緩解期與急性期組相比,ALFF增加的腦區為右側殼覈,ALFF降低的腦區為直迴、楔前葉、梭狀迴/舌迴、左側顳葉、左側中央前迴和後迴、左側枕中迴。結論:精神分裂癥患者早期階段存在多箇腦區靜息態低頻振幅異常,低頻振幅降低的腦區主要分佈在左側大腦半毬。
목적:통과정식태공능자공진탐토조기정신분렬증환자뇌공능저빈진폭변화。방법:대수차발병미용약조기정신분렬증환자(급성기조)、경약물치료림상증상완해적조기정신분렬증수차발병환자(완해기조)이급성별、년령、수교육년한필배적건강대조자(대조조)각17례,진행정식태공능자공진소묘,병계산저빈진폭( ALFF),비교3조ALFF변화。결과:삼조간ALFF유차이적뇌구위좌측직회、사상회/설회、우측섭하회、좌측정협、침협、설협、중앙후회、소뇌;여건강대조조상비,량병례조ALFF강저적뇌구집중재좌측반구,위좌측정협소협,좌측섭하회;이완해기여급성기조상비,ALFF증가적뇌구위우측각핵,ALFF강저적뇌구위직회、설전협、사상회/설회、좌측섭협、좌측중앙전회화후회、좌측침중회。결론:정신분렬증환자조기계단존재다개뇌구정식태저빈진폭이상,저빈진폭강저적뇌구주요분포재좌측대뇌반구。
Objective:to explore brain function alteration in patients of schizophrenia in early stage. Method:enrolled 17 patients of schizophrenia in early course with first episode drug-native( acute group),17 antipsychotics treated patients of schizophrenia with first episode( remission group),and 17 sex,age,education years matched healthy subjects( control group)underwent resting-state functional magnetic resonance image( fM-RI)scan. Amplitude of low-frequency fluctuation( ALFF)of every voxel of every subject was calculated and ana-lyzed among three groups in one-way ANOVA. Results:Significant abnormal ALFF were found in multiple re-gions such as left recuts( LR),fusiform/lingual gyrus( FG/LG),right inferior temporal gyrus( ITG),left parietal lobe,occipital lobe,cuneus,postcentral gyrus,cerebellum. Compared with control group,all two patient groups had decreased ALFF regions mostly in left hemisphere,including left inferior parietal lobe and left ITG. Com-pared with acute group,remission group had increased ALFF regions in right putamen,but decreased ALFF re-gions in R,precuneus,FG/LG,left TG,left middle occipital gyrus,left precentral/postcentral gyrus. Conclusion:schizophrenia patients in early stage may have abnormal ALFF in multiple regions,and decreased ALFF regions maybe mostly distribute in left hemisphere.
