目的:探讨经椎旁肌间隙入路结合伤椎置钉治疗胸腰椎骨折的应用效果。方法52例无神经损伤的胸腰椎骨折患者,依据治疗措施不同随机分为对照组(22例,传统开放入路)和观察组(30例,经椎旁肌间隙入路结合伤椎置钉),观察对比两组治疗效果。结果观察组手术时间、术中出血量、术后引流量均优于对照组,观察组胸腰椎骨折患者伤椎Cobb角纠正率和腰背痛视觉模拟评分(VAS评分)均优于对照组,差异均有统计学意义(P<0.05)。结论经椎旁肌间隙入路结合伤椎置钉治疗胸腰椎骨折的应用效果明显,并发症少,值得临床推广应用。

목적:탐토경추방기간극입로결합상추치정치료흉요추골절적응용효과。방법52례무신경손상적흉요추골절환자,의거치료조시불동수궤분위대조조(22례,전통개방입로)화관찰조(30례,경추방기간극입로결합상추치정),관찰대비량조치료효과。결과관찰조수술시간、술중출혈량、술후인류량균우우대조조,관찰조흉요추골절환자상추Cobb각규정솔화요배통시각모의평분(VAS평분)균우우대조조,차이균유통계학의의(P<0.05)。결론경추방기간극입로결합상추치정치료흉요추골절적응용효과명현,병발증소,치득림상추엄응용。
Objective To investigate the application effect of paravertebral intermuscular approach combined with injured vertebral nailing in the treatment of thoracolumbar vertebral fracture.Methods A total of 52 thoracolumbar vertebral fracture patients were divided by different treatment measures into control group (22 cases, traditional open approach) and observation group (30 cases, paravertebral intermuscular approach combined with injured vertebral nailing). Curative effects were compared between the two groups.Results The observation group had better operation time, intraoperative bleeding volume, and postoperative drainage volume than the control group, and it also had better injured vertebral Cobb angle correct rate and lumbago and backache visual analogue scale (VAS) score than the control group. The difference had statistical significance (P<0.05). ConclusionCombination of paravertebral intermuscular approach and injured vertebral nailing can provide obvious effect in treating thoracolumbar vertebral fracture with few complications. This method is worthy of clinical promotion and application.
