目的：探讨应用内侧辅助入路技术关节镜下处理外侧半月板前角水平层裂的疗效。方法2011年1月至2012年12月，应用内侧辅助入路关节镜下治疗15例外侧半月板前角水平层裂患者。采用3个入路：前外侧入路、前内侧入路、内侧辅助入路，经前内侧入路置入关节镜观察，于前外侧入路使用探钩将外侧半月板前角挑起，经内侧辅助入路置入篮钳行前角的成形及层裂的处理。结果15例获10～26个月随访，平均16.6个月。12例膝关节完全不痛，3例偶尔运动后疼痛不适。术后活动范围均恢复至正常。膝关节 Lysholm 评分由术前(70.1±8.3)分，术后(92.7±4.8)分(配对t 检验，t＝7.45，P＝0.05)。结论关节镜下应用内侧辅助入路技术，处理外侧半月板前角损伤，简单有效，临床效果满意。
목적：탐토응용내측보조입로기술관절경하처리외측반월판전각수평층렬적료효。방법2011년1월지2012년12월，응용내측보조입로관절경하치료15예외측반월판전각수평층렬환자。채용3개입로：전외측입로、전내측입로、내측보조입로，경전내측입로치입관절경관찰，우전외측입로사용탐구장외측반월판전각도기，경내측보조입로치입람겸행전각적성형급층렬적처리。결과15례획10～26개월수방，평균16.6개월。12례슬관절완전불통，3례우이운동후동통불괄。술후활동범위균회복지정상。슬관절 Lysholm 평분유술전(70.1±8.3)분，술후(92.7±4.8)분(배대t 검험，t＝7.45，P＝0.05)。결론관절경하응용내측보조입로기술，처리외측반월판전각손상，간단유효，림상효과만의。
Objective To evaluate the curative results of arthroscopic treatment for horizontal tears in the anterior horn of the lateral meniscus through an assistant medial approach.Methods From January 2011 to December 2012, 23 patients with horizontal tears in the anterior horn of the lateral meniscus underwent arthroscopic treatment using an assistant medial approach. Anterolateral, anteromedial and assistant medial appraoches were employed in the patients. An arthroscope was placed via the anteromedial approach. The superior leaf of the anterior horn was averted by a probe through the anterolateral approach, and the inferior leaf of the anterior horn was excised with a punch through the assistant medial approach.Results All the 15 patients were followed up for a mean period of 16.6 months ( range: 10-26 months ). Twelve patients had knee joint pain before the operation, which disappeared completely. The other 3 patients occasionally had knee joint pain after doing sports. The range of motion ( ROM ) of the knee joints recovered to a normal level after the operation, and the Lysholm scores were increased from ( 70.1±8.3 ) points preoperatively to ( 92.7±4.8 ) points postoperatively ( pairedt test,t=7.45,P=0.05 ).Conclusions Assistant medial approach provides a safe and effective alternative for arthroscopic treatment for horizontal tears in the anterior horn of the lateral meniscus.
