目的：探讨平阳霉素、地塞米松、利多卡因混合局部注射联合手术切除治疗头颈部海绵状血管瘤的临床价值。方法：回顾性分析58例头颈部海绵状血管瘤患者的临床资料，确诊后首先应用平阳霉素、地塞米松、利多卡因混合局部注射，待瘤体明显缩小，质地韧硬，边界清楚后再行手术切除。结果：所有患者瘤体切除均＞90％，皮肤和皮瓣移植均成活。术后均未出现明显的组织器官的缺损或畸形。随访1～8年，平均2年，3例患者复发后再次入院治疗，其余患者均未见复发，治愈率为94．8％。结论：平阳霉素、地塞米松、利多卡因混合局部注射联合手术切除头颈部海绵状血管瘤可以达到彻底治愈和改善畸形的目的，疗效令人满意。
목적：탐토평양매소、지새미송、리다잡인혼합국부주사연합수술절제치료두경부해면상혈관류적림상개치。방법：회고성분석58례두경부해면상혈관류환자적림상자료，학진후수선응용평양매소、지새미송、리다잡인혼합국부주사，대류체명현축소，질지인경，변계청초후재행수술절제。결과：소유환자류체절제균＞90％，피부화피판이식균성활。술후균미출현명현적조직기관적결손혹기형。수방1～8년，평균2년，3례환자복발후재차입원치료，기여환자균미견복발，치유솔위94．8％。결론：평양매소、지새미송、리다잡인혼합국부주사연합수술절제두경부해면상혈관류가이체도철저치유화개선기형적목적，료효령인만의。
Objective:To explore the clinical value of Local injection with pingyangmycin、dexamethasone and lidocaine combined with surgical resection of head and neck cavernous hemangioma .Methods:We retrospectively analyzed the clinical data of 58 cases of head and neck cavernous hemangioma .Local injection with pingyangmycin、dexamethasone and lidocaine was administrated in all patients once the diagnosis was confirmed .Surgical resection was performed when the tumor shrank with clear boundary and became strong and pliable . Results::More than 90%of tumor was resected for every patient .All transplanted skin and flap survived .Neither obvious defect nor de-formity of the tissues and organs occurred postoperatively .All patients were followed up for 1 to8 years ( mean 2 years) .3 patients were hospitalized again because of tumor recurrence during the follow -up period.The total cure rate is up to 94.8%.C onclusion:Local injec-tion with pingyangmycin、dexamethasone and lidocaine combined with surgical resection can cure head and neck cavernous hemangioma and improve the deformity caused by the tumor .Our experience may justify the satisfactory outcomes .
