目的：探讨开放式完全腹膜外疝修补术（TEP）对术后慢性疼痛的影响。方法采用随机双盲对照法，对2013年1～12月昆明医科大学第二附属医院120例腹股沟疝患者，分别采用传统前入路腹膜前间隙术式（55例）和开放 TEP 术式（65例）进行疝修补治疗，并统计二组患者术后发生慢性疼痛例数。结果传统前入路腹膜前间隙组术后慢性疼痛发生率为10．9％（6／55），TEP 组术后慢性疼痛发生率为1．5％（1／65），二组比较差异有统计学意义（χ2＝4．76，P ＝0．029）。结论开放式TEP 可避免术中对腹股沟区神经损伤，减少术后慢性疼痛发生率。
목적：탐토개방식완전복막외산수보술（TEP）대술후만성동통적영향。방법채용수궤쌍맹대조법，대2013년1～12월곤명의과대학제이부속의원120례복고구산환자，분별채용전통전입로복막전간극술식（55례）화개방 TEP 술식（65례）진행산수보치료，병통계이조환자술후발생만성동통례수。결과전통전입로복막전간극조술후만성동통발생솔위10．9％（6／55），TEP 조술후만성동통발생솔위1．5％（1／65），이조비교차이유통계학의의（χ2＝4．76，P ＝0．029）。결론개방식TEP 가피면술중대복고구구신경손상，감소술후만성동통발생솔。
Objective To explore the influence of chronic pain after open totally extraperitoneal herniorrhaphy (TEP).Methods By the randomized double blind method,1 20 cases of inguinal hernia in Second Affiliated Hospital of Kunming Medical University were treated with either open totally extraperitoneal herniorrhaphy (n =65)or traditional anterior approach (n =55).Demographic data were recorded,and chronic postoperative pain were evaluated with the visual analog scale.Results The incidence of chronic pain was 1 .5% (1 /65)in open TEP group and 1 0.9% (6 /55)in the traditional anterior approach group. There was a significant difference between the two groups (χ2 =4.76,P =0.029).Conclusion Open TEP can avoid the inguinal region nerve injury and significantly reduce the incidence of chronic pain.
