目的:探讨双源 CT 血管造影(CTA)在颅内动脉瘤术前评估中的应用价值。方法回顾性分析43例疑诊颅内动脉瘤的蛛网膜下腔出血患者的双源 CT 血管造影图像,并与数字减影血管造影(DSA)或手术所见对照。结果43例中 CTA 检出37例39个动脉瘤,6例阴性。 CTA 清晰显示动脉瘤的部位、大小、形态、指向、瘤颈、瘤壁钙化、瘤内血栓、载瘤血管及与周围组织的关系等病变细节。25例(CTA 阳性19例和阴性 6例)行 DSA 检查,检出20例20个动脉瘤,5例阴性。共21例颅内动脉瘤患者接受外科手术治疗。除漏诊 1例 <3 mm 的动脉瘤外,CTA 所示动脉瘤细节与 DSA 或术中所见完全一致。结论双源 CT 血管造影能快速诊断颅内动脉瘤并提供动脉瘤及颅内情况的详实信息,为术前评估提供重要指导性依据。
目的:探討雙源 CT 血管造影(CTA)在顱內動脈瘤術前評估中的應用價值。方法迴顧性分析43例疑診顱內動脈瘤的蛛網膜下腔齣血患者的雙源 CT 血管造影圖像,併與數字減影血管造影(DSA)或手術所見對照。結果43例中 CTA 檢齣37例39箇動脈瘤,6例陰性。 CTA 清晰顯示動脈瘤的部位、大小、形態、指嚮、瘤頸、瘤壁鈣化、瘤內血栓、載瘤血管及與週圍組織的關繫等病變細節。25例(CTA 暘性19例和陰性 6例)行 DSA 檢查,檢齣20例20箇動脈瘤,5例陰性。共21例顱內動脈瘤患者接受外科手術治療。除漏診 1例 <3 mm 的動脈瘤外,CTA 所示動脈瘤細節與 DSA 或術中所見完全一緻。結論雙源 CT 血管造影能快速診斷顱內動脈瘤併提供動脈瘤及顱內情況的詳實信息,為術前評估提供重要指導性依據。
목적:탐토쌍원 CT 혈관조영(CTA)재로내동맥류술전평고중적응용개치。방법회고성분석43례의진로내동맥류적주망막하강출혈환자적쌍원 CT 혈관조영도상,병여수자감영혈관조영(DSA)혹수술소견대조。결과43례중 CTA 검출37례39개동맥류,6례음성。 CTA 청석현시동맥류적부위、대소、형태、지향、류경、류벽개화、류내혈전、재류혈관급여주위조직적관계등병변세절。25례(CTA 양성19례화음성 6례)행 DSA 검사,검출20례20개동맥류,5례음성。공21례로내동맥류환자접수외과수술치료。제루진 1례 <3 mm 적동맥류외,CTA 소시동맥류세절여 DSA 혹술중소견완전일치。결론쌍원 CT 혈관조영능쾌속진단로내동맥류병제공동맥류급로내정황적상실신식,위술전평고제공중요지도성의거。
Objective To assess the application of dual -source CT angiography (CTA) for preoperative assess-ment of intracranial aneurysm.Methods Images were collected from 43 subarachnoid hemorrhage patients with suspec-ted intracranial aneurysm who underwent dual -source CTA.These data were retrospectively analyzed and then compared with those obtained through digital subtraction angiography (DSA) and surgical findings.Results Using CAT, 39 out of 43 patients with suspected aneurysm were diagnosed, with 6 negative cases.CTA could clearly display the pathological changes of aneurysm in details such as location, size, shape, pointing, aneurysmal neck, calcification of the aneurysmal wall, intra -aneurysmal thrombosis, aneurysm -bearing vessels and their relationship with surrounding tissues.There were also 25 patients (including 19 CAT positive and 6 negative patients) who underwent DSA, in which 20 CAT positive (with 20 aneurysms) and 5 negative patients were determined.Furthermore, a total of 21 aneurysm patients were treated by surgery.The pathological details of aneurysms which were reflected through CTA were consistent with those obtained through DSA or surgical findings, except one case where an aneurysm less than 3 mm was missed diagnosed.Conclusion CTA can be applied for quick diagnosis of intracranial aneurysm, while showing detailed information about aneurysm and its surroundings, which then provides important evidence for preoperative assessment.
