目的：：分析原发性低颅压综合征( spontaneous intracranial hypotension,SIH)的MRI征象,提高对该病的诊断水平。方法：回顾性分析14例经临床证实的SIH的MRI表现。结果：本组14例SIH患者MRI表现包括：平扫未见明显异常者2例(14．29%)；硬膜下积液7例(50．00%)；硬膜下血肿2例(14．29%)；脑下垂8例(57．14%)；垂体增大4例(28．57%)；静脉窦扩张5例(35．71%)；硬脑膜弥漫性增厚强化14例(100．00%)。结论： SIH的MRI征象中,硬脑膜弥漫增厚强化的阳性率最高,对本病的诊断和鉴别诊断具有较大价值。

Objective:Analyze the MRI manifestations of spontaneous intracranial hypotension ( SIH) to improve the diagnosis of the disease. Methods:MRI manifestations of 14 patients clinically confirmed SIH were retrospectively analyzed. Results:Of all 14 ca-ses’ MRI manifestations,2 cases (14. 29%) had no obvious abnormality in scanning;subdural fluid accumulation showed in 7 cases (50. 00%);subdural hematoma in 2 cases ( 14. 29%);sagging of brain in 8 cases ( 57. 14%);pituitary hyperemia in 4 cases (28. 57%);engorgement of venous structures in 5 cases (35. 71%);enhancement of meninges in 14 cases (100. 00%). Conclusion:Enhancement of meninges has the highest positive rate in all MRI signs,and has greater value for the diagnosis and differential diagnosis of spontaneous intracranial hypotension.
